We are looking to recruit a part time registered nurse to work with us covering maternity leave within the Amber Valley Rapid Response team. We are currently based at Babington Hospital but soon to move into our brand new Health Centre!  There is scope for flexibility with hours/working patterns for the right candidate.

Main duties of the job

The role includes managing referrals into our service and providing evidence-based care to support patients in their own home.

To assess, plan, implement and evaluate individualised patient care, promoting self care wherever possible in an efficient and clinically effective way, whilst maintaining privacy and dignity.

To provide advice and support to junior staff in the delivery of high quality care and actively support the senior Community  Nurses / District Nurse in the management of the health care environment.

During you days you will be working alongside general practice/therapy/district nursing colleagues  and  services within the acute  hospitals to prevent hospital admission and support discharge home.
