We are looking for a qualified nurse to join our North Hardwick Core Community District Nursing Team to complement our current staff. The post could also involve working across other teams in North East Derbyshire

Main duties of the job

Provide high-quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home.

Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way, whilst maintaining the patient’s privacy and dignity.

Provide advice and support to junior staff in the delivery of high quality care and actively support the District Nurse in the management of the health care environment.

Implement / refer on to the relevant services as appropriate.

Work in close collaboration with the Integrated Community Teams/GP surgeries and local acute trusts.

Education / Qualifications

Essential

  • NMC registered Nurse (including newly-qualified and soon-to-be-qualified)
  • Evidence of recent relevant training

Experience and Knowledge Required

Essential

  • Experience of working in the community
  • Evidence of research based/reflective practice
  • Knowledge of local and national priorities.
  • Understanding of the wider NHS agenda regarding community services

Desirable

  • Experience of long term conditions
  • Experience of Leg Ulcer management and lower limb assessments
