Main duties of the job

As a Community Staff Nurse, you will:

• Deliver high-quality, person-centred nursing care to patients in their own homes and community settings.
• Assess, plan, implement and evaluate care to meet individual patient needs.
• Work in partnership with patients, carers and multidisciplinary colleagues to coordinate effective care and support.
• Promote the highest standards of care, compassion and professionalism in everything you do.
• Support the coordination of the service during the short-term absence of the District Nurse.
• Work closely with GPs, practice teams, palliative care services, social care teams and voluntary agencies to ensure seamless patient care.
• Support and encourage colleagues, learners and support workers within the team.
• Contribute to service development, quality improvement initiatives and the advancement of community nursing practice.
• Maintain and develop your clinical knowledge and skills through ongoing professional development.
