We are seeking a motivated and compassionate Band 5 Community Psychiatric Nurse to join our established Locality Mental Health Team in Skegness, providing high‑quality, recovery‑focused care to adults with a range of complex mental health needs.

This is an excellent opportunity for a newly qualified or early‑career mental health nurse who is passionate about community practice and eager to develop clinical skills. You will work as part of a supportive multidisciplinary team, delivering evidence‑based interventions, contributing to holistic assessments, and supporting safe and effective care planning.

We are looking for someone who is passionate about community mental health and recovery‑focused practice and able to build therapeutic relationships with service users and carers.

You will be a reflective practitioner committed to learning and professional growth and a team player with strong communication and organisational skills.

As a Band 5 nurse, you will receive structured supervision, preceptorship, and development opportunities to build confidence in autonomous practice, risk management, and care coordination.

Main duties of the job

Comprehensive assessment — Undertake biopsychosocial assessments, formulation, and risk assessments for adults referred to the service.

Case management — Manage an allocated caseload, developing person‑centred care plans in line with the LMHT SOP and Framework.

Risk management — Contribute to dynamic risk assessment, crisis planning, and safety interventions.

Therapeutic interventions — Deliver evidence‑based interventions, including psychological, psychosocial, and recovery‑focused approaches.

Duty work — Participate in the team’s duty rota, responding to urgent referrals and supporting crisis presentations.

Medication Management - Administer depot medications in the community and support service users prescribed clozapine medication in line with policy and guidance and with support of MDT.

Multidisciplinary collaboration — Work closely with psychiatry, psychology, social care, primary care, and voluntary sector partners.

Clinical documentation — Maintain accurate, timely, and professional records in line with Trust and NMC/HCPC standards.

Service development — Contribute to quality improvement, transformation initiatives, and trauma‑informed, recovery‑oriented practice.

Supervision and support — Provide supervision, mentorship, and support to junior staff and students.

Safeguarding — Uphold safeguarding responsibilities for adults and children, working within statutory frameworks.
