An opportunity has arisen with the service, for a Band 5 Learning Disability Nurse post to work within the Specialist Community Learning Disability Team and innovative. The Learning Disability Service strives to provide forward thinking high quality services, working in partnership with other agencies. In addition to working with children with learning disabilities and autism, we work closely with their families, and we have a strong commitment to health promotion through service development and training initiatives. We are committed to helping our staff develop and you will be part of an enthusiastic and experienced team.

The post holder will work within an integrated health team, providing clinical support to children with learning disabilities who have additional complex needs, which may include challenging behaviour, physical and sensory impairment, mental health and forensic issues. The post holder will play an integral role in delivery of care within these pathways within a variety of community settings.

Main duties of the job

As a band 5 community nurse you will be expected to support the clinical activity of the teams in meeting the needs of local people with learning disabilities in the community. You will have skills in working with children with learning disabilities, and associated communication and behavioural needs. The post holder will undertake delegated duties by senior nurses, and will be accountable to the Operational Manager and Senior Community Nurse and Locality Lead Nurse, and will be providing administrative support within the integrated health team, to ensure effective and timely clinical outcomes are met.
