22.5 hours per week.

The Community Nurse is responsible for assessing, planning, coordinating, and reviewing a caseload of adults with learning disabilities. They may have either or both physical and mental health complex care needs. This will include relationships with Primary and Secondary health care, as well as private and voluntary care agencies.

The Community Learning Disability Service covers Plymouth City offering a specialist service to Adults with a Learning Disability registered to a Plymouth GP surgery and comes under the City-Wide Locality Services.

The service is led by the Service Manager and 2 Group Managers and professional leads. We focus on Multi-Disciplinary/Professional led care. The team provides Consultant Psychiatry care alongside Psychology, Nursing (learning Disability and Mental Health), Speech and Language, Occupational Therapy, Dietician, Positive Behavioural Support, Health Education and Promotion, Primary Care Liaison Nurses and Community Support Workers.

Main duties of the job

To be responsible for providing clinical support whilst promoting the effective and efficient delivery of high-quality outcome focused service in a person-centred approach.

This is a generic community care role providing support and cover across Plymouth city localities.

There is responsibility for participating in reviewing a persons eligibility for Continuing Healthcare funding and is responsible for making consistent and well-informed professional decisions about the determination of CHC funding, ensuring that the national framework is applied, and they adhere to all local policies and procedures.

This role may not be eligible for sponsorship under the Skilled Worker route; please refer to the Direct Gov website for more information with regards to eligibility.

Job responsibilities

Clinical Responsibilities

  • Participating in the undertaking of health needs reviews and health assessments to identify needs and plan care accordingly.
  • To identify, plan and manage risk of an individuals care needs, relating to physical health, behavioural and or mental health.
  • To be responsible for assessing, planning and making recommendations for future care in partnership with patient, carer and multi-professional agencies including case conferences for reviewing the packages of care and care co-coordinating as appropriate.
  • To liaise with Primary and Secondary care agencies to ensure better health outcomes for individuals.
  • To work within the Mental Capacity Act and evidence best interest decision making.
  • To maintain effective and appropriate communication networks with members of the multi-disciplinary team, the independent sector homes and all stakeholder agencies and organisations to ensure that the individual care needs can be met.
  • To contribute to multi-disciplinary team specialist assessments.
  • Ability to work as an autonomous practitioner, planning, prioritising and organising workload according to the needs of the service.
  • To provide any relevant information and explanation to clients and carers at a level commensurate with their ability to understand using total communication.
  • To provide evidence-based advice and guidance to care staff on the management of health needs to patients and signpost to other services.
  • To act as a supervisor and/or assessor for students demonstrating high standards of practice and professional conduct.
  • To undertake health promotion/education whenever possible to all adults with LD and also to contribute to planned health promotion activities.

  • To maintain a high professional standard of conduct at all times in accordance with NMC/ professional bodies code of professional conduct. Maintain an active status on NMC/ professional bodies register.

COMMUNICATIONS AND RELATIONSHIPS

  • The post holder will be required to work within the MDT function of the CLDT and may need to be part of wider MDT groups relating to client need.
  • The post holder will liaise between other health, social and educational services in assessing, planning and reviewing client care needs/service provision.
