<p><strong>Make a meaningful difference every day by delivering expert care that enables patients to remain safe, independent, and supported within their own homes and communities.</strong></p><p>As a Band 6 Community Nurse / District Nurse, you will play a key role in delivering high-quality, person-centred care within the community setting. Working as part of a multi-professional team, you will be responsible for the assessment, planning, coordination, and delivery of clinical care that meets the needs of patients and the service.</p><p>You will work autonomously within your scope of practice, using your clinical expertise, professional judgement, and evidence-based knowledge to make informed decisions and deliver safe, effective care. As a registered practitioner, you will be accountable for your actions, recognising the limits of your competence and seeking support where appropriate.</p><p>The role also includes supervising and supporting junior staff, support workers, and students, helping to develop future healthcare professionals. You will actively contribute to quality improvement initiatives, service development, and the ongoing enhancement of patient outcomes, ensuring care is delivered in line with professional standards, organisational values, and best practice.</p><p><strong>Location:</strong> County Hall.
