Rosedean House Surgery is seeking an experienced Community Matron to join our primary care team. This position is ideal for a clinician who is passionate about proactive, patient-centred care, enjoys working across a diverse community, and wants to make a meaningful difference in the lives of both older adults and younger adults living with complex health needs.

Main duties of the job

We are proud to have recently been rated Good by the CQC and remain committed to continuous improvement as we further develop and enhance our services. The Community Matron will play a central role in:

  • Enabling patients to remain independent and at home for as long as possible

  • You will be someone who enjoys building strong, trusting relationships with patients and their families.
  • Experience in supporting older adults including frailty, geriatrics, care homes, or complex long-term conditions is highly valued, while also recognising the broader needs of adults of working age.
