We are seeking a dedicated and compassionate Community Learning Disabilities Nurse to join our multidisciplinary team. This is an exciting opportunity to make a real difference in the lives of people with learning disabilities, supporting them to achieve better health outcomes and live fulfilling lives in the community.

Role;

  • Provide specialist nursing care and health facilitation for individuals with learning disabilities.
  • Undertake comprehensive health assessments and develop person-centred care plans.
  • Work collaboratively with families, carers, and other professionals across health and social care.
  • Deliver health promotion, education, and training to service users, carers, and staff.

Requirements

  • Registered Nurse (RNLD or RN with relevant experience).
  • Evidence of post-registration learning and professional development.
  • Experience working with people with learning disabilities in community settings.
  • Strong communication, leadership, and problem-solving skills.
  • Ability to work autonomously and as part of a team.

Main duties of the job

  • To provide specialist support, advice and guidance to individuals, families and carers.
  • To provide specialist support as part of the multi-disciplinary team
  • To complete specialist assessments as clinically indicated
  • To participate in PBS training and offer specialist support within the PBS framework
  • To participate in referral meeting, team meetings and wider MDT meetings.
  • To contribute to team and service development.
  • To carry an individual caseload.
  • To keep up-to-date with research and development in your fields of practice
  • To attend training relevant to the role
  • To maintain NMC registration and remain up-to-date with research and development in learning disability nursing practice
  • To complete individual risk assessments under supervision and to put a safety plan in place if needed.
  • To communicate with and support social workers, care providers and other agencies and services.
  • To supervise and assess student nurses on placement with the team.

Main Duties/Responsibilities

Leadership:

  • To practice within the NMC Code of Professional Conduct.
  • To be professionally accountable for the effective and efficient management of a defined caseload, taking into account the need to combine your own responsibilities and those of the multi-disciplinary team.
  • To be responsible for setting high standards of care within the resources available and to review and monitor these on a continuous basis through the process of audit, case review and on-going supervision.
  • To promote a positive approach to health education and health promotion.
  • Participate in consultancy and specialist input to a range of other professionals and agencies in relation to individuals with complex needs.
  • Advise and influence local and regional policy, strategy and operational practice in relation to health needs of people with learning disabilities.
