A substantiative full-time post is available within the community heart failure team. We are seeking to recruit one individual who has relevant knowledge, skills and experience in all aspects of holistic nursing.
A key element of the role is to assess and support senior heart failure specialist nurses to implement and evaluate care for patients with chronic and complex heart failure. The successful candidate will play a pivotal role in providing evidence-based care in all aspects of heart failure.
The individual should have excellent communication and organisational skills, be able to work automatously and work as part of a dedicated team of heart failure nurses.
Main duties of the job
This is an exciting opportunity for a registered nurse who has a passion for heart failure to join the highly motivated Sandwell community heart failure team. The team aims to promote the delivery of high quality, patient focused evidence based practice. The team objectives are aligned to national and local agreed quality agendas and the promotion of safe harm free care.
