Main duties of the job
As a Community Health Navigator, you will play a crucial role in our service, helping to connect individuals with the appropriate care they need. Situated in the BSW (Bath & North East Somerset, Swindon, and Wiltshire) Community Health Services Single Point of Access (SPA), your role will require you to be the first point of contact for various stakeholders, including patients, healthcare professionals, and members of the public. - Be the welcoming first point of contact for all SPA enquiries
- Support callers with empathy and professionalism, identifying needs and directing them to the right services
- Handle sensitive and sometimes complex information with confidentiality and care
- Keep accurate, up-to-date patient records across clinical systems
- Process referrals and correspondence efficiently and accurately
- Book, amend and manage clinical appointments in line with service targets
- Escalate urgent or incomplete referrals appropriately to clinical teams
- Provide essential admin support to keep services running smoothly
- Work collaboratively with a friendly, supportive team, contributing to shared goals

Working patterns
The SPA operates from 8:00am to 8:00pm, seven days a week, 365 days a year.We offer a range of flexible shift patterns to accommodate service needs while supporting a healthy worklife balance.
- GCSEs (or equivalent) in Maths and English at Grade AC.
- Previous administrative experience.
- Confident use of IT systems, with strong keyboard skills and attention to accuracy.
- Good working knowledge of Microsoft Office (Outlook, Word, Excel).
- Strong communication and interpersonal skills, with the ability to communicate clearly both verbally and in writing.
- Ability to work with sensitivity, discretion and confidentiality.
- Wellorganised, with the ability to prioritise tasks, meet deadlines and work effectively in a busy environment.
- Able to work well independently and as part of a team.
- NVQ Level 2 in Administration and/or Customer Services (or equivalent).
- Previous experience within health or social care.
- Knowledge of medical terminology.
- Experience using clinical systems or databases.
