<p><strong>Be the First Point of <span class="single_desc_int_gb_word">Support</span> That Makes a Difference</strong></p><p>We are looking for motivated and customer-focused <strong>Community Health Navigators</strong> to join our BSW (Bath & North East Somerset, Swindon and Wiltshire) Community Health Services <strong>Single Point of Access (SPA)</strong> team.</p><p>As a Community Health Navigator, you will play a vital role in helping patients access the right care and <span class="single_desc_int_gb_word">support at the right time</span>. Acting as the first point of contact for patients, members of the public, healthcare professionals and colleagues, you will provide a professional, <span class="single_desc_int_gb_word">compassionate and efficient service across a range of community health services</span>.</p><p>Working within a busy and <span class="single_desc_int_gb_word">supportive team, you will handle enquiries via telephone, email and post, providing information, guidance and signposting to appropriate services</span>.
