Join Devon Partnership NHS Trust as a Clinical Team Manager (MDT) within our Specialist Learning Disability Team in East Devon and help shape high quality, person-centred care for adults with a learning disability. This is a pivotal leadership role where you'll combine clinical expertise with operational management supporting a multidisciplinary team to deliver safe, effective and evidence-based interventions across key functions including IATT, CHC and Liaison.

You'll lead on performance, quality and workforce development, build strong partnerships with families, carers and agencies, and drive improvements that reduce health inequalities and prevent avoidable hospital admissions through the Transforming Care agenda. Main duties of the job

The post holder will be responsible for the effective operational management of a Multidisciplinary Team (MDT) including line management of staff, financial and performance management. They will use effective leadership, change management and team building skills to monitor, evaluate and develop the service provided.

The MDT will provide a range of the current functions which include IATT, CHC and Liaison.

The post holder will be responsible to support across all Specialist Learning Disability (LD) functions.

In conjunction with the Senior Management Team/and Operational Manager for the MDT, the post holder will support development of the MDT service in their area of responsibility, to become a centre of excellence and expertise in delivering specialist service to adults with a LD. This will include ensuring inappropriate hospital admissions are avoided through applying the Transforming Care Agenda.

To ensure delivery of high quality care and treatment through evidenced based practice.

To provide clinical advice and support to the MDT, enabling them to undertake assessments in order to meet the needs of adults with LD who are on the caseload.

Ensuring services are driven by the voices of the people who use them. This involves communicating with and working in partnership with a range of clinicians and services, working to change systems and educating where appropriate. It also involves close communication and involvement with family, carers and other stakeholders.
