Are you a registered nurse seeking an opportunity to work as a Community Specialist Palliative Care Nurse in domiciliary and care home settings?

We are looking for an experienced nurse with a passion for Palliative Care to join our community Specialist Palliative Care service across the Bradford area.

In this role, you will provide clinical case management to patients nearing the end of life, supporting them in their own homes or care homes. You’ll focus on improving their health and well-being, offering clinical assessments and treatments to ensure comfort and dignity at the end of life. By doing so, you will help reduce unnecessary hospital admissions, ease the demand on GP services, and contribute to delivering timely, efficient, and high-quality care.

Main duties of the job

  • Assessing, planning, and providing specialist care and support to individuals with life-limiting illnesses, including end-of-life care.
  • Collaborating effectively within a multidisciplinary team, working alongside colleagues, patients, caregivers, and other health and social care professionals, as well as statutory, voluntary, and independent organizations, to optimize patient management.
  • Ensuring adherence to statutory legislation and local policies relating to discharge planning and palliative care.
  • Managing your own caseload autonomously and providing face-to-face services seven days a week.
  • Acting as a resource, educator, and advocate for patients, their families, and healthcare professionals.
  • Contributing to staff education initiatives.
  • Participating in audit activities to support continuous improvement.
