
Are you a registered nurse seeking an opportunity to work as a Community Specialist Palliative Care Nurse in domiciliary and care home settings?
We are looking for an experienced nurse with a passion for Palliative Care to join our community Specialist Palliative Care service across the Bradford area.
In this role, you will provide clinical case management to patients nearing the end of life, supporting them in their own homes or care homes. You’ll focus on improving their health and well-being, offering clinical assessments and treatments to ensure comfort and dignity at the end of life. By doing so, you will help reduce unnecessary hospital admissions, ease the demand on GP services, and contribute to delivering timely, efficient, and high-quality care.
