Due to the expansion of the metabolic bone service we have an exciting opportunity for a clincial nurse specialist with experience in metabolic bone disorders to join our service.

The clinical nurse specialist in metabolic bone disorder will have good knowledge and experience of metabolic bone disorders and will work closely with the DMOPS consultant to provide specialist care to patients with metabolic bone disorders. The post holder will play a key role in the Metabolic bone disorders service and will take a lead on developing a nurse led service.

Main duties of the job

• Develop standardised pathways for assessment and treatment of patients with metabolic bone disorders, specifically osteoporosis
• Lead quality improvement projects to increase efficiency in the metabolic bone disorders clinic
• Develop new workstreams including CNS-led osteoporosis clinic
• Lead CNS osteoporosis clinic under the care of DMOPS consultant
• Liaise with DMOPS colleagues across the wards and FAU to educate and develop quality improvement projects for patients with osteoporosis
• Monitor re admission rates of patient with metabolic bone disorders and be a key part in admission avoidance of these patients
• Work cross-site with our Royal colleagues in clinical chemistry to reduce health inequity
• Manage a case load of patients under the care of a name DMOPS consultant
• If required Refer patients to various disciplines post assessment for ongoing care and support.
• The Clinical Nurse Specialist will act as the clinical expert and professional resource to the multi-Professional team, patients and carers within their Speciality Service.
• Monitor and audit effectiveness of the service by measuring outcomes pre and post induction including length of stay, quality outcomes and morbidity/mortality

Detailed job description and main responsibilities

• Develop, lead and support Osteoporosis nurse-led clinic
• Triage of referrals to appropriate service in appropriate timeframe
• Preparation of metabolic bone advice and guidance requests
• Provide in depth assessment of patients undergoing comprehensive treatment for metabolic bone disorders and create ongoing care plans based on this assessment
• Lead fracture liaison service pilot
• Utilise clinical skills in assessing and treating patients as an autonomous practitioner
• Monitor investigation results, initiating further assessment/review of treatment pathway as required
• Collect, collate, evaluate and report information, maintaining accurate patient records
• Involve patients and carers/relatives in the planning and delivery of care and development of services.
• Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery
• Act as the patients advocate and ambassador.
• Lead the development of a patient-focussed education and information giving strategy within the speciality
• Contribute to the health promotion agenda with the local health economy.
• To undertake/work towards non-medical prescribing duties within the legal framework for nurse prescribing and adhere to the Trust’s Policy for Non-medical prescribing (where required)
• In keeping with the NMC ‘Code of Professional Conduct’ and ‘Standards of Proficiency for Nurse and Midwife Prescribers’, keep up to date with best prescribing practice in relation to the management of conditions within his/her specialist area
• Undertake regular audits of his/her prescribing practice
• Provide specialist expert clinical advice and guidance to clinical colleagues
• Undertake clinical supervision on an individual or group basis.
• Recognise, prevent and diffuse potential areas of conflict and take a key role in the management of the outcome
• Act as a role model and expert nurse providing in depth specialist clinical knowledge to colleagues through MDT meetings, for patients and carers / relatives
• Engage in research projects within scope of practice
• Lead research projects (if appropriate) relevant to expert subject and implement changes to practice based on findings
• Assess local and national patient satisfaction results of service and use this information to implement and monitor change to practice to improve patient experience
• Contribute to the organisation and care groups clinical governance agenda
• Participate in local and national audits for metabolic bone disorders and use findings to implement and sustain changes to practice
• Maintain knowledge and update clinical policy and practices based on up-to-date national research and innovation
• Attend conferences within clinical expertise and feed back to wider team to promote innovation and change to practice
• Be consistent in approach to the provision of evidence-based care for patients with metabolic bone disorders by providing evaluation of interventions and treatments
• Develop and seek funding for projects which will enhance, promote and develop the service provision
• Disseminate research and audit findings through presentation to professional groups and publication.
• Identify need for, participate and undertake, research, clinical audit, benchmarking and equipment trials, in order to improve effectiveness of patient care.
• Be responsible for the development and co ordination of the service including identifying areas for improvement and work within trust guidance and protocol to address these areas, monitor and improve
• Promote co-ordination of multi-disciplinary working within the service
• Lead on community referrals into the service
• Manage own workload ensuring working within KPIs such as roster and annual leave
• Manage budget if appropriate
• Work within organisations values and behaviours
• Ensure necessary cover/plans for patient reviews in the absence of the metabolic bone CNS
• Follow trust policy and procedures
• Provide cover and undertake duties of absent colleagues if appropriate
• Work within own competence and limitations
• Attend conferences to develop own skill and the service
• Participate in local and national forums for metabolic bone disorders
• Participate in networking groups to develop the service and develop relationships with other organisations
• Ensure yearly appraisal is completed
• Work within NMC guidelines and code of professional conduct including revalidation completion and maintaining professional registration
• Completion of mandatory, local and role specific training
