The post holder will be an integral member of the Hub Bed and Chartridge Multidisciplinary Team (MDT) and provide proactive clinical complex case work for patients within the Community Discharge Hubs, Chartridge Ward, and Interim Health Funded (IHF) beds as required.
To ensure and promote the timely access to health and social care resources and work in partnership with other MDT and interdependent teams to ensure a safe and timely discharge from the service.

To work with a range of professionals in order to ensure care is appropriate and delivered in a timely manner. The post holder will be able to demonstrate their professional skills and competence in leadership, care planning / co-ordination, medicines management, and care and treatment delivery.
Main duties of the job
To provide pro-active and responsive support to the Clinical Operational Lead for patients in the Hub Beds, Chartridge Ward, and Interim Health Funded beds (IHF) as required.
To work across all Hub Beds and where appropriate IHF sites, BHT Hospital and other community hospital sites to meet the needs of an efficient Community Discharge Hub Service.
The case management model will provide interventions tailored to meet the needs of the individual.

EXPERIENCE
Essential
- Demonstrate experience post or pre-registration placements in relevant clinical areas
- In depth experience of working within multi-agency/multi-disciplinary team
Desirable
- Experience of working with single assessment process
- Experience of electronic clinical systems
- Experience participating in research or audit project
- Experience of day-to-day staff management e.g. case allocation to Health Care Assistants
SPECIAL CIRCUMSTANCES
Essential
- Valid and current driving licence and car to be able to travel to locations throughout Buckinghamshire
