Clinical Lead - Rapid Assessment Psychosis Team (RAPT)

Job overview

We are seeking to recruit an experienced, motivated and innovative clinical lead within the Rapid Assessment Psychosis Team (RAPT). The post holder will provide day to day clinical leadership to ensure timely, safe and effective delivery of care to service users and their families across Greenwich, Bexley and Bromley boroughs.

They will:

• Provide clinical leadership, direction and robust clinical decision making across the psychosis teams.
• Ensure the delivery of high-quality care by continually developing the service and improving standards.
• Oversee the delivery of evidence based clinical interventions for service users and their families.
• Work alongside staff across the pathway to support and develop their clinical skills and experience.
• Build and maintain the positive reputation of the team and service.

Main duties of the job

• Provide clinical supervision and consultation to a range of professionals across the psychosis pathway.
• Provide clinical leadership and expertise to staff, leading day to day clinical decision making
• Lead/ chair clinical team meetings and forums including referrals, zoning, complex case discussions, reflective practice and discharge planning.
• Enable staff across the pathway to complete person centred clinical risk assessments and formulations and oversee development of robust safety plans.
• Deliver evidence based NICE concordant interventions for people experiencing psychosis and their families.
• Lead the team to develop proactive and safe discharge plans for service users.
• Develop and maintain partnership working arrangements with a wide range of statutory and voluntary organisations.
• Work with service users and families in a variety of settings, including the person’s home and a range of community settings.
• Be directly accountable for own professional action, quality of clinical intervention provided and the maintenance of high standards of care within professional codes of practice, professional ethics and legal standards.
• Develop therapeutic relationships with service users and families and support the team to engage flexibly and creatively to allow access to services and treatments.
• Lead the team to provide culturally appropriate services and interventions for service users and families.

Detailed job description and main responsibilities

Management

• Co-ordinate all care and treatment delivered to the service user within the clinical area, optimising all available resources.
• Ensure that a culture of effective risk management is present in the team.
• Ensure all aspects of Safeguarding are adhered to and that policies and procedures are followed at all times.
• Formulate and implement local clinical and operational practices and procedures as appropriate
• Provide effective and autonomous management oversight for a caseload of service users under the care of the psychosis pathway.
• Plan, organise and prioritize own workload effectively, ensuring that appropriate time and attention is allocated to different aspects of responsibility, i.e. clinical management, supervision and teaching.
• Communicate in a skilled and sensitive manner as part of an integrated service culture where there is professional respect between disciplines.
• Ensure that Trust policies and procedures are understood and adhered to by all staff within their scope of responsibility.
• Participate in the recruitment and selection process in accordance with Trust policy.
• Ensure effective induction of staff in accordance with Trust policy.
• Investigate complaints and incidents in accordance with Trust policy.
• Undertake Serious Untoward Incident investigations/reviews as required.
• Ensure that all staff are regularly updated and have knowledge of all emergency procedures.
• Active participation in multidisciplinary decision-making process regarding patient care in the capacity of an autonomous clinician.
• Ensure the team Use resources and equipment appropriately, safely and economically ensuring duty of care at all times.
• Ensuring clinical space complies with Trust policies and procedures.
• Oversight for ordering and maintaining stock required for clinical activities (PPE etc).
• Effective management of risk of infection ensuring related polices fully complied with and communicated in the team(s).

Service Development:

• Actively participate in psychosis pathway policy reviews.
• Establish implement and review patient care pathways/protocols and practice guidelines.
• Ensure the principles of Clinical Governance are incorporated in service delivery by maintaining a high-quality service based on agreed clinical standards.
• Contribute to the monitoring and evaluation of interventions across the psychosis pathway.
• Ensure clinical outcomes are recorded and shared for monitoring purposes.
• Contribute to the consultation and engagement of service users, carers and families in planning and delivering services which meet the needs of local communities.
• Contribute to strategic forums as directed.

Leadership

• Act as a role model in encouraging and inspiring individuals and the team to deliver evidence-based interventions and good quality care.
• Engage and facilitate staff to work collaboratively in providing NICE concordant interventions.
• Create a condition that enables the individual team member to perform at their best – by providing encouragement, support, advice and clinical supervision.
• Lead and ensure staff participation in the formulation of new ideas/ suggestions to improve service user care and service delivery.
• Review and reflect on own practice and perform through effective use of management and professional supervision.
• Develop own and others knowledge and practice across professional and organisational boundaries.

IT responsibilities.

• To regularly enter and update information regarding clinical activity into the RiO clinical records system.
• To competently utilise Microsoft packages including Word, Excel and PowerPoint in order to produce professional clinical and training materials for use as part of normal clinical responsibilities.

Clinical recordkeeping

• To ensure that up to date written and electronic records and activity data are maintained in accordance with Professional and Trust standards.
• To comply with Trust reporting procedures for incidents and complaints.

Research and development

• Support the development of a skilled and flexible workforce, supporting junior staff to achieve their full potential.
• Engage, support and supervise multi professional staff in providing evidence-based interventions.
• Facilitate group supervision sessions/learning forums for team members.
• Update and maintain specialist skills and knowledge relevant to working in the psychosis pathway.
• Attend specialist supervision provided for the delivery of NICE concordant intervention.
• Attend all mandatory training as directed by the Trust.
• Ensure own Continuing Professional Development in line with professional body, professional registering body and Trust Personal Development Plan requirements.
• Comply with the relevant professional standards of conduct, ethics and proficiency, and ensure professional development in line with these.
• Keep up to date with relevant legislation and local and national NHS policy
• Participate in research and audit projects
• Demonstrate the ability to critically evaluate current research, apply them to practice and disseminate finding at a local level.

• Be regularly able to work under pressure and stressful situations, managing conflicts, managing competing demands, prioritising effectively, taking measured risks making decisions and judgement
• Be aware of own strengths and limitations and be able to manage own anxiety and appear confident to staff and clients providing daily leadership to the team.
• Manage high anxiety and emotions generated by regularly working with service users who may be at a serious risk to themselves or others – managing own feelings and those of the client.
