Evelina London is at the heart of a network of children’s health services in Lambeth and Southwark and across South-East England. We provide a comprehensive range of children’s health services, so that we are able to look after patients from antenatal diagnosis through childhood, into adolescence and on into adult services.

The post holder will be responsible for facilitating early discharge, reduce length of stay and prevent hospital admissions.  We are seeking enthusiastic experienced paediatric nurses, who have a passion for acute care in the community, to work as part of the existing team to support  an exciting new pilot in digital health technology for remote monitoring on our virtual ward.

You will work as an autonomous practitioner assessing patients’ needs and using evidenced based clinical skills alongside digital technology to care for patients in their own home. You will also be responsible for caseload management, handover of care and service developments providing specialist input when necessary.

Car driver essential. Pool cars (inc automatic) are provided for team use.

Main duties of the job

The successful candidate will join the established Children’s at Home team within our Children’s Community Nursing Service, in the delivery of quality care for children and young people. This is an exciting opportunity to be part of a new development working with patients in their own home with high acuity needs. This will be integral in enabling patients to achieve optimum function and receive appropriate treatment within their own home that will prevent unnecessary admission to hospital.

You will be supported by the senior clinical team including ACPs, trust doctors/consultants, respiratory physiotherapists, and senior pharmacists. There will be daily board round meetings held to discuss the patients cared for on the Virtual Wards and a weekly MDT meeting.

We will provide the successful candidate with the relevant in-house training and further education supported by our education department.  You will be enrolled on a community development pathway that will include Evelina London Foundations in Children’s Community Care (EL FiCC) and Evelina London Paediatric enhanced assessment  course(El Peca)

Previous experience of working in a community setting and/or working autonomously with acute nursing skills is essential for this post.

Detailed job description and main responsibilities

In this role you will work with a high degree of autonomy, demonstrating core capabilities in clinical competency. You will treat patients with acute exacerbations of chronic diseases and acute presentations in the assessment, planning, and treatment of patients to prevent them being admitted to hospital. You will have the ability to triage and prioritise  the needs of patients, making any referrals for further investigations where appropriate in line with local and trust guidelines, whilst ensuring your clinical practice is safe and effective,  escalating  appropriately to senior colleagues when needed. You should have excellent clinical, written, and verbal communication skills, be able to work independently and manage your own workload.

The job role is community based across Boroughs of Lambeth and Southwark and works collaboratively with Primary, secondary and emergency care providers.

• The post holder will be an autonomous practitioner, continually developing a highly specialised knowledge base and clinical assessment skills, working within agreed protocols.
• The post holder will deliver, implement, monitor, and escalate patients on the Evelina Children’s at Home virtual ward and where appropriate liaising with medical colleagues in order to assess, implement and modify treatment or to facilitate timely reassessment of patients on the service.
• They will ensure appropriate pathways of care and communication via liaison and referral to other agencies and specialist teams via the MDT as required.
• The post Holder will ensure compliance with Professional policies, National and local guidelines and Trust policies, procedures, and protocols ensuring they work within their scope of practice and clinical competencies.
• The post holder will also play a key role in the delivery and development of formal and informal education to members of the multi professional team.
• Will utilise advanced knowledge and skills and provide appropriate care, clinical reasoning skills and evidence-based practice to provide specialist advice and support for individuals or the multi professional team in the care of patients within the service.
• Work as an autonomous practitioner; without direct supervision, drawing on advanced theoretical and practical experience.

Clinical and Patient Care Independently manage a group of patients within the specialty whilst working as part of the multidisciplinary team, delivering specialised individualised and personalised direct patient care.

Be responsible for assessing and recognising emergencies in the speciality. To interpret information and take appropriate action.

Demonstrate appropriate practice and specialist advice in the assessment, planning, implementation and evaluation of patient care. This includes history taking, physical examination and the requesting of agreed diagnostic procedures/clinical investigations.

Responsible for the correct administration of prescribed medication to ensure safe custody of medications in the home setting.

Be responsible for assessing and recognising emergencies in the speciality. To interpret information and take appropriate action.

Act as an advocate for patients, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines.

Ensure that patients’ receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines.

Recognise and avoid situations that may be detrimental to the health and wellbeing of the individuals.

Recognise and monitor acute exacerbations of illness and initiate appropriate nursing interventions.

Is wholly accountable for his / her practice in line with the NMC code of professional conduct and takes every reasonable opportunity to sustain and improve his / her knowledge and professional competence.
