Childrens Community Nurse - band 6
Part time 23.75 hrs per week
The post‑holder will provide high‑quality, specialist community nursing care to children and young people with acute, chronic, complex and palliative health needs. Care will be delivered in homes, schools, clinics, and other community settings, ensuring children can access education, daily activities, and safe care closer to home.
The role includes responsibility for managing a defined caseload, delivering evidence‑based nursing interventions, supporting parents/carers as partners in care, and acting as a key health professional for children with complex health conditions in educational settings.
The post‑holder will contribute to the delivery of a fully integrated Children’s Community Nursing Team, supporting service development, quality improvement, safeguarding, and multi‑agency collaboration. They will also participate in the 24‑hour on‑call rota for end‑of‑life care.

Main duties of the job
Assess, plan, implement and evaluate programmes of care for children requiring nursing support at home or in community settings.
Provide specialist nursing interventions including gastrostomy care, accessing central lines, suctioning, medication administration, and long‑term condition management.
Support inclusion of children with health needs in nurseries, schools, and community activities.
Contribute to palliative and end‑of‑life care, including participation in the on‑call rota.
Follow all safeguarding policies and procedures, ensuring concerns are escalated appropriately.
Promote seamless care between hospital and community services.
Promote effective teamwork and support colleagues.

- RN Child/RSCN
- Mentorship course or equivalent
- Current NMC registration – child branch
- Evidence of recent Continuous Professional Development
