An exciting opportunity has arisen for motivated individual to work within the Community Nursing Division as a Community Nursing Caseload Manager, within the Wallasey Locality working with The New Brighton community nursing team for maternity leave cover. Main duties of the job
- hours per week, fixed term for 10 months maternity leave cover. Hours and shift patterns to be discussed at interview.
This post will be responsible for managing planned programmes of care for patients in domiciliary and clinic settings.
A main part of the role of a Caseload Manager is to assess, plan, implement and evaluate nursing care to patients across the community/primary care interface in the most efficient and clinically effective way, in co-operation with other members of the health and social care teams, and voluntary and statutory agencies.

You will need to be able to work autonomously and as part of a team and possess excellent time management, organisational and communication skills. You will be innovative, flexible and prepared to adapt to the changing needs of the Trust as they arise.
