Main duties of the job

We are looking for a positive and motivated Carer Coordinator, ideally with lived experience of caring for someone with mental and/or physical ill health and/or learning disability, to join our well-established Patient and Carer Experience and Involvement Team.  With proven experience of leading on involvement and co production projects involving carers across health and/or care services, you will be clearly visible across our teams and services, supporting staff to work together with carers, to introduce positive changes to carer experiences. You will have experience of using a trauma informed approach to guide your work and will have experience of working with carers from a diverse range of backgrounds.

You will support teams and services to work towards achieving the 6 standards of the Triangle of Care Framework, a set of standards introduced by the Carers Trust which support our staff to ensure they are meaningfully involving and effectively supporting carers.  You will be responsible for identifying mechanisms to collect, collate and report back on carer feedback to ensure that carers voices are heard and acted on by senior decision makers at a Trust, regional and national level.

Together with the Experience Coordinator within the Patient and Carer Experience Team, you will also oversee the organisation and coordination of the Trusts thriving Service User Network (SUN) meeting which is made up of service users, carers and members of the public who have an interest in helping to improve Trust services.  You will encourage carers from a diverse range of backgrounds, to get involved.
