Care Navigator / Administrator

Full-time - Permanent

Monday - Friday, 0900-1730

Are you a confident communicator with excellent customer service skills and a passion for helping people?

Path House Medical Practice is looking for an enthusiastic and motivated Care Navigator to join our friendly and supportive team. Serving approximately 17,300 patients, we are a busy, forward thinking practice committed to delivering high quality patient care.

About the Role

A Care Navigator is the vital first point of contact for patients accessing our services. Working in a fast paced environment you will help patients access the right care, from the right person, at the right time.

Please note that a significant proportion of this role involves telephone work. Successful candidates will spend much of their working day answering a high volume of patient calls, assessing patients needs, booking appointments and ensuring patients are directed to the most appropriate member of the healthcare team.

Applicants should be confident communicators with an excellent telephone manner and the ability to remain professional, compassionate and efficient in a busy environment.

The role requires excellent communication skills, empathy, sound judgement and the ability to maintain strict confidentiality.

Key Responsibilities

· Answering and managing incoming patient calls

· Booking, rearranging and cancelling appointments over the phone using clinical software

· Greeting and booking in patients at reception

· Booking, rearranging and cancelling appointments using clinical software

· Assessing patient needs and signposting to the most appropriate clinician or service· Processing repeat prescription requests

About You

You will:

·Have an excellent telephone manner and strong customer service skills

· Enjoy communicating with people and be comfortable spending much of the day on the telephone

· Demonstrate empathy, professionalism and discretion

· Be able to work calmly and effectively in a busy environment

· Have good IT skills and the ability to learn specialist GP systems

· Work well both independently and as part of a team Experience in a healthcare, reception, customer service or administrative role would be advantageous but is not essential.

Experience of EMIS and Docman would be beneficial, although full training will be provided.

Working Hours 37.5 hours per week.

The successful candidate will work 7.5 hours per day, Monday to Friday. Shifts will be scheduled between Practice opening hours of 08:00 and 18:00 and may vary according to service requirements.
