<p><strong><em>Make a real difference to families and young people in Lancashire</em></strong></p><p>HCRG Care Group is looking for a <span class="single_desc_int_gb_word">compassionate, organised and customerfocused <strong>Care Coordinator (Administrator)</strong> to join our Care Coordination Hub</span>. This is a fantastic opportunity to be part of a <span class="single_desc_int_gb_word">supportive team helping children, young people and families access the right care at the right time</span>.</p><p><strong>What youll do</strong></p><p>As the first point of contact for patients, parents, carers and professionals, you will play a key role in delivering an excellent customer experience. In this role, you will:</p><ul><li><strong>Provide a friendly, efficient and professional <span class="single_desc_int_gb_word">response</span></strong> to all incoming telephone calls and emails into the Care Coordination Hub.</li><li><strong>Filter, prioritise and direct queries</strong> to the correct Lancashire Healthy Young People & Family Services teams.</li><li><strong><span class="single_desc_int_gb_word">Support</span> clinical teams</strong> by booking and scheduling routine clinic appointments in line with service rules
