Job summary

We are looking for a dedicated and motivated Care Co-Ordinator / Team leader to join our existing team.

Ideally full-time, however we are open to applications from applicants looking for 30+ hours per week.

The post holder is responsible for leading and co-ordinating the smooth and efficient running of all work delegated to our team of care co-ordinators and developing colleagues to support the provision of high quality and responsive administrative services for patients.

The post holder is responsible for the day-to-day administrative work within the team under the supervision of the PCN Operational Manager.

The post holder should have a passion for providing a quality service and a track record of adding value to a workplace.

Main duties of the job

Ensure provision of efficient and effective administrative services within the PCN.

Ensure all workstreams are completed to a high standard and remain on target.

Holgate Primary Care Network (PCN) serves a population of circa 44,000 patients residing in Middlesbrough.

We are a forward thinking, innovative PCN with a large clinical and administrative team.

We work collaboratively with other partners to strengthen and support the delivery of Primary Care services, our PCN consists of 3 core GP practices Borough Road & Nunthorpe Medical Group, Linthorpe Surgery and Village Medical Practice. The PCN is led by our Clinical Director, Dr P Heywood.

Job responsibilities

Main Responsibilities and Duties, include:

Ensure all workstreams all completed to a high standard and remain on target, workstreams include (but are not limited to):

Delivery of EiCH

Learning Disabilities

Cancer Care

Dementia

Vaccinations and Immunisations

Flu and Covid Vaccination Programme

Urgent Care Hub

Hypertension

Population Health Management

Ensure adequate staff cover is always maintained.

Ensure all daily, weekly and monthly tasks connected to all workstreams are completed in a timely manner.

To support the Operational Manager in the training and induction of new staff.

Ensure the team have completed all mandatory training annually when due.

Dealing with more complex queries from patients or other team members.

Provide a link between the care co-ordinators and operational manager.

Using GP Systems (SystmOne) to identify and record patient information.

Working closely with PCN Operational Manager to establish processes for the care co-ordinator workstreams.

Maintain relationships with practices and promote communication.

Liaise with IG lead to ensure correct coding and data entry.

Liaise with Safeguarding lead.
