Job summary

An exciting opportunity has arisen for a Band 5 Bone Health Nurse to join Nottingham CityCare Partnerships Bone Health Service on a 12-month fixed term contract or 12-month secondment.

We have a full time position and a part time position available (working 22.5 hours per week)

This is a community-based role within a proactive, forward-thinking service delivering evidence-based assessment, treatment, and follow-up for individuals with osteoporosis and those at risk of fragility fractures. The successful applicants will join a supportive multidisciplinary team and contribute to the delivery of high-quality, person-centred care.

The role involves venepuncture, cannulation, and the administration of injection and IV medication in patients own homes.

Working pattern:

Service hours are Monday to Friday, 08:00 - 16:00.

The team is based at Bulwell Riverside, with the service covering Nottingham City and South Nottinghamshire County. The team offers a supportive environment with opportunities to learn, develop, and grow within community nursing.

Main duties of the job

• Deliver evidence based osteoporosis assessment, treatment, and fracture prevention care within a community setting,
• Support the assessment, monitoring, education, and follow up of patients requiring ongoing bone health management.
• Undertake agreed clinical procedures in line with training and competency, including venepuncture, injections, cannulation, and administration of IV medication.
• Provide patient centred education and advice to promote self management, treatment adherence, and fracture risk reduction.
• Maintain accurate, timely clinical documentation in line with professional, legal, and organisational requirements.
• Work effectively as part of the multidisciplinary team, liaising with colleagues across community, primary, and secondary care.
• Recognise clinical concerns, risks, or deterioration and escalate appropriately in accordance with local protocols.
• Participate in service delivery, audit, and quality improvement activities within the Bone Health Service.
• Engage in mandatory training, supervision, appraisal, and continuing professional development.
• Support the learning and development of student nurses and learners.

Job responsibilities

Job Purpose

• To provide assessment, treatment, and ongoing management for patients with osteoporosis and other bone health conditions under the care of the Nottingham CityCare Bone Health Specialist Service.
• To work autonomously to deliver high-quality, evidence-based care to patients in their own homes, care homes and other community settings, ensuring care is tailored to individual needs.
• To facilitate, coordinate and independently deliver osteoporosis treatments within community-based settings, in accordance with local and national guidelines.
• To work collaboratively with General Practitioners, Community Geriatricians and Specialist Secondary Care Clinicians to support the assessment, management and ongoing care of patients with osteoporosis and other bone health conditions across Nottingham City and South Nottinghamshire County, ensuring a coordinated and patient-centred approach to care.
• To contribute to fracture prevention and bone health promotion through patient education, clinical monitoring and the implementation of evidence-based interventions.
• To act as a specialist resource for patients, carers and healthcare professionals, providing advice and support relating to osteoporosis and bone health management.

Dimensions

• The post holder will be part of the Nottingham CityCare Bone Health Specialist Service, providing care to patients across Nottingham City and South Nottinghamshire County.
• The primary focus of the role will be the administration of intravenous and injectable osteoporosis medication within patients own homes and care home settings. The post holder will spend most of their time undertaking direct patient-facing clinical activity and will work closely with the Bone Health Caseload Administrator to coordinate patient care and treatment delivery.
• The post holder will undertake agreed clinical procedures in line with training, competency and local policies, including venepuncture, cannulation, administration of injectable and intravenous medications.
• The post holder will be professionally supported by the Bone Health Nurse Specialists, who will provide clinical guidance and support in the management of more complex cases.
• The post holder will be required to meet the physical demands of the role, including undertaking manual handling activities in accordance with mandatory training requirements and organisational policies.
• The post holder will be required to travel throughout Nottingham City and South Nottinghamshire County to deliver clinical care, attend meetings and support service provision. Occasional travel beyond these areas may be required to meet the needs of the service.
• The post holder will maintain accurate clinical records and contribute to service evaluation, clinical audit, quality improvement initiatives and the monitoring of patient outcomes in accordance with organisational policies and professional standards.

Key Responsibilities

Clinical

• To be professionally accountable for all aspects of clinical practice, ensuring the delivery of safe, high-quality, evidence-based care in accordance with professional standards, organisational policies and national guidance.
• To work autonomously within patients own homes, care homes and other community settings to assess, plan, implement and evaluate individualised care.
• To undertake agreed clinical procedures in line with training and competency, including venepuncture, cannulation, administration of injectable treatment and intravenous medications.
• To accept responsibility for the management of a designated caseload, prioritising workload effectively to meet patient needs and service requirements.
• To monitor patients receiving osteoporosis treatments, identifying, managing and escalating adverse effects, clinical concerns or deteriorations as appropriate.
• To participate in Bone Health virtual clinics and multidisciplinary discussions alongside Consultant Specialists and the wider Bone Health Team.
• To provide education, advice and support to patients, carers and healthcare professionals regarding osteoporosis, bone health and treatment options.
• To demonstrate effective communication skills when assessing, educating and obtaining informed consent from patients with a wide range of health, cognitive and social care needs.
• To undertake assessments of mental capacity, obtain valid informed consent, and work within the legal and ethical framework of the Mental Capacity Act and associated legislation when supporting patients who may lack capacity to make decisions regarding their care and treatment.
• To communicate clinical information clearly and sensitively to patients, carers and other healthcare professionals, adapting communication style to meet individual needs and maximise understanding.
• To ensure that patients, families and carers are appropriately involved in decision-making and care planning, promoting person-centred care and shared decision-making wherever possible.
• To maintain accurate, timely and comprehensive clinical records in accordance with professional, organisational and legal requirements.
• To ensure the health, safety and wellbeing of patients, carers and colleagues, adhering to Health and Safety legislation and organisational policies.
• Identify, report and manage risks appropriately, including the completion of In Phase incident reports and implementation of learning from incidents.

Professional

• To demonstrate a commitment to evidence-based practice and seek specialist advice and support from Bone Health Nurse Specialists when managing complex cases.
• To maintain professional registration and practice in accordance with the Nursing and Midwifery Council (NMC) Code and relevant professional standards.
• To be responsible for own Continuing Professional Development (CPD), maintaining up-to-date knowledge of developments, research and best practice within osteoporosis and bone health care.
• To attend and participate in relevant internal and external education, training events, conferences and professional networks as appropriate to the role.
• To contribute to the development of clinical practice through reflection, learning and the implementation of evidence-based care.
• To support, supervise and assess nursing students and other learners as required, acting as a practice assessor and/or supervisor in accordance with professional standards.
• To act as a positive ambassador for the Bone Health Service and Nottingham CityCare Partnership, promoting professional standards and organisational values.

Leadership and Management

• To support clinical audit, service evaluation and quality improvement activities.
• To contribute to the ongoing development of the Bone Health Service by identifying opportunities for improvement and supporting change initiatives.
• To provide supervision, support and mentoring to staff members and students as appropriate.
• To support effective multidisciplinary and multi-agency working, promoting collaborative relationships across primary, secondary and community care services.
• To contribute to team meetings, service developments and organisational initiatives
• To demonstrate leadership within the scope of the role, acting as a role model for professional practice and supporting a culture of continuous improvement.
