POST: Rapid Response Domiciliary Care Liaison Officer (Community Discharge Service)

RESPONSIBLE TO: Community Discharge Service Assistant Service Manager
The purpose of The Community Discharge Service is to facilitate safe discharge on the appropriate pathway for patients who have been declared medically fit within a time frame of 48 hours.
The purpose of this service is to enable safe and appropriate earlier discharge for in- patients from the acute hospitals thereby reducing length of stay for in-patient admissions.
The service is rapidly developing and with current demands on capacity in domiciliary care the BHSCT is exploring avenues to work differently. As part of the Community Discharge Service the BHSCT are working with independent providers to provide a rapid response service to facilitate hospital discharges.
The post-holder will be the primary liaison with independent providers who will provide a rapid response for hospital discharges and will be required to coordinate referrals from the acute hospitals to the independent sector to coordinate the discharges.
The post holder within Community Discharge Service, will communicate with the all relevant stakeholders to ensure a safe and compassionate discharge.
Respond to referrals from acute hospitals and interim facilities for care provision to support discharge.
Be the key BHSCT liaison with independent providers affiliated to the project
Complete relevant documentation required by independent providers to facilitate a safe discharge
Facilitate weekly meetings with independent providers to monitor feedback and performance.
To undertake the monitoring of service provision to service users and record service users’ satisfaction to ensure a quality service delivery and to participate in any audit of service provision.
To ensure that service users and prospective service users are provided with information on services provided and all service users have individual service agreements and individual Community Discharge Service care plans.
To ensure that all duties undertaken in relation to the service users community Discharge Service Care plan are recorded and relevant information communicated to the appropriate people.
To liaise with family and carers of service user as appropriate.
Under the direction of the Community discharge service coordinator participate in the development and implementation of standards, which will ensure the delivery of a quality service.
Collate information on hospital discharges and referrals to independent sector and validate information supplied by independent sector.
Preparation and collation of all statistical information required for internal and external stakeholders.
Ensure the accuracy and timeliness of data inputs and appropriate outputs for client based Computer Systems.
Establish monthly validation of all computer based and manual records.
To participate in the investigation of complaints in line with the Trust’s complaints procedure.
To undertake risk management training and ensure adherence to health and safety policies and procedures.
To ensure appropriate reporting of accidents and incidents and undertake investigations where appropriate.
To ensure completion and maintenance of agency records as required.
To maintain confidentiality in accordance with the Code of Confidentiality and the Data Protection Act.
To be aware of the Human Rights Legislation in relation to the requirements of this post.
Any other duties as and when required.

Infection Prevention and Control
The Belfast Trust is committed to reducing Healthcare associated infections (HCAIs) and all staff have a part to play in making this happen. Staff must comply with all policies in relation to Infection Prevention and Control and with ongoing reduction strategies. Standard Infection Prevention and Control Precautions must be used at all times to ensure the safety of patients and staff.

Cleaning hands either with soap and water or a hand sanitiser at the appropriate times (WHO ‘5 moments’);
Using the correct ‘7 step’ hand hygiene technique;
Being ‘bare below the elbows’ when in a clinical environment;
Following Trust policies and the Regional Infection Control Manual (found on intranet);
Wearing the correct Personal Protective Equipment (PPE);
Ensuring correct handling and disposal of waste (including sharps) and laundry;
Ensuring all medical devices (equipment) are decontaminated appropriately i.e. cleaned, disinfected and/or sterilised;
Ensuring compliance with High Impact Interventions.
