JOB DESCRIPTION

POST: Community Support Worker Band 3

LOCATION: Recovery Mental Health- Various BHSCT

Grade: Band 3

REPORTS TO: Team Leader Community Mental Health Support Team

RESPONSIBLE TO: Service Manager Adult Mental Health

Job Summary / Main Purpose

The Community Support Workers offer support to individuals with mental health difficulties, in order to maximise their ability to live independently in their own homes. This will involve visitng service users in their own homes and encouraging them to utilise community services.

Main Duties / Responsibilities

1. To work as a member of the multi disciplinary team

2. To provide support to enable Service Users to maintain their existing tenancies in the community.

3. To support Service users to maintain their current tenancy by providing housing support

4. To keep all relevant records up to date and accurate.

5. Advice and support service users in the maintenance of good standards of home care and budgeting skills and advice in the management and security of their homes. Where necessary accompany and support Service users whilst shopping or whilst engaged in other community activities.

6. To work as part of the multi disciplinary team in partnership with service users and carers in planning support and in the development of individual support plans.

7. To support directly an individual or small group to participate in community based activities.

8. To assist service user's overcome problems they might experience whilst participating in community activities, and to accompany and assist service user's in developing an awareness of their locality / local community, thereby promoting community involvement.

9. To enable service user's to increase their capacity for independence and independent decision making.

10. To be available to listen and talk to service users when required.

11. Notify the key worker of any change in the service user's circumstances and maintain all records as instructed by the key worker and report any untoward incident or complaint, or abuse of a vulnerable adult.

12. To contribute to the maintenance of recording on PARIS

13. Assisting service users with filling in forms i.e. Housing Benefit, PIP Forms etc.

14. Assist service users in every detail of their daily living, advising to seek appropriate services, psychiatrists, dentists, doctors, podiatrists etc.

15. Accompany Service Users to outpatients appointments as required.

16. Observe all health and safety regulations, observe correct use of electrical equipment, ensure fire safety procedures are adhered to where applicable and support service users in the reporting procedures for all faults, as and where they require to be passed on to the relevant authority.

17. Attend staff meetings and supervision sessions as required.

18. Contribute to Multidisciplinary meeting as required.

19. Attend and contribute to review meetings of service users upon the request of key workers.

20. To liaise with the Team Leader on all leave entitlements and adhere to the policies and procedures of the Trust.

22. To be aware of the Human rights legislation in relation to the requirements of this post.

23. To comply with statutory regulations, e.g. in relation to health and safety and Service User related legislation.

Environmental Cleaning Strategy

Infection Prevention and Control

The Belfast Trust is committed to reducing Healthcare associated infections (HCAIs) and all staff have a part to play in making this happen. Staff must comply with all policies in relation to Infection Prevention and Control and with ongoing reduction strategies. Standard Infection Prevention and Control Precautions must be used at all times to ensure the safety of patients and staff.

Cleaning hands either with soap and water or a hand sanitiser at the appropriate times (WHO '5 moments');

Using the correct '7 step' hand hygiene technique;

Being 'bare below the elbows' when in a clinical environment;

Following Trust policies and the Regional Infection Control Manual (found on intranet);

Wearing the correct Personal Protective Equipment (PPE);

Ensuring correct handling and disposal of waste (including sharps) and laundry;

Ensuring all medical devices (equipment) are decontaminated appropriately i.e. cleaned, disinfected and/or sterilised;

Ensuring compliance with High Impact Interventions.

PERSONNEL SPECIFICATION

Community Support Worker Band 3Team Leader Community Mental Health Support Team

5 GCSE's level C or above to include Maths and English

or

Equivalent or higher educational qualification

NVQ level 2 in a care related subject

Or

18 months caring experience in a residential/Nursing/Community/Day Care/Voluntary sector

And

Must undertake the QCF Level 2 Diploma in Health and Social Care within 12 months of commencing post.

Registration, or evidence of initiating the process of Registration, with the NI Social Care Council.

One years experience working with adults with mental health difficulties.

Hold a current U.K. driving licence with access to a car. This Criterion will be waived in the case of an applicant whose disability prohibits driving but who is able to organise suitable alternative arrangements.

All applicants must be able to work a flexible shift pattern, which will include working evenings, weekends and bank and public holidays. (Enhancements are payable) The service is operational 9am - 8pm 7 days a week.Shortlisting by Application Form

NOTE
