We are currently recruiting Band 6 Bed Managers for our Acute Care Coordination Centre (ACCC). The post is based at Springfield Hospital.

We are seeking to appoint a highly motivated nurse or equivalent clinician to be part of a Team that is at the heart of the Acute and Urgent Care Service Line in managing bed capacity and demand. As well as keeping the flow of patients moving through our acute wards, you will also have a key role in managing any operational issues out of hours that might affect the three hospital sites and can impact the safe and efficient functioning of the service. This is a full-time post and an exciting opportunity for an experienced Band 5 or Band 6 clinician who is seeking a different challenge.

This is an exciting time to join the team as we expand and move to a fully computerised system that will in the future allow us to predict and prepare for future demand. The successful candidates will be actively involved in service development and improvement.

This opportunity will provide you with an excellent opportunity to develop your skills in management, leadership, problem-solving as well as decision-making and interagency working.

SWLSTG In-patient MDTs (Consultants and Ward staff and pharmacists) and other inpatient bed providers

Discharge Coordinators

SWLSTG community clinical teams and Care Coordinators, Home Treatment, Psychiatric Liaison, CMHT/RST, Early Intervention, Older adults and Specialist Services Complex needs/ rehab service/forensic, Acute Trusts Site Managers, CSU Surge colleagues, Other NHS Trusts,  AMHPs, Coral Crisis Hub, Police, Liaison and Diversion Practitioners

Service Users, Carers and friends and family

Heads of Nursing & Quality, Heads of Service Delivery Matrons and Clinical Service Leads

Through the Daily Staffing Meeting, Pathway Meeting, Sit Rep Meeting, Oncall Breach update meeting

Primary Care clinicians

Detailed job description and main responsibilities

Bed Co-Ordinators to work in conjunction with internal and external partners in supporting proactive coordination of bed capacity, in line with the Clinical Prioritisation Framework and escalating lack of capacity and breaches to support timely access to care.

To provide accurate information for the daily discharge, pathway, and other relevant capacity and breach meetings

To update the Bed Waiting List in order to accurately reflect referrals for assessment, referrals for bed, sources, location, breach times and other, relevant data to support prioritisation.
