Job overview

The Adult Community Nursing provides nursing care provision and support to Hounslow locality 24 hours, 7 days a week. The service covers different localities including Heart of Hounslow, Bedfont and Feltham, Brentford and Chiswick and the night service from 7pm-7am based at Clayponds Hospital.

Main duties of the job

  • To work with the locality nursing team leadership in the delivery of the locality nursing service in line with the commissioned service specification, standards, activity and outcomes, in the context of multi-disciplinary locality working.
  • To support the locality nursing team leadership in the management of the team caseload and the development of a skilled, competent and motivated workforce, able to meet the needs of the patients on the caseload.
  • To work in partnership with the primary health care team, social services and the wider multi-disciplinary locality team, providing holistic assessment of health needs and the provision of nursing care to housebound people with a range of medical needs and long-term conditions.

Experience

  • Recent experience of Nursing at Level 5, or evidence of a back to nursing programme Two years’ experience of working in the community setting
  • Two years worked post registration experience in a relevant setting, e.g. palliative care, care of the older person, orthopaedics, community services
  • Supervision of junior staff

Qualifications

Essential

  • Level 1 Registered Nurse- with valid registration with the NMC - RN1 ,RNA
  • Experience or recognised qualification in areas such as:  Health Promotion  Diabetes  Care of the older person  Continence  Palliative care  Wound management Medical/Surgical Nursing

Skills and Abilities

Essential

  • Ability to undertake a shift system
  • Good communication skills effective with clients and colleagues
  • Able to work without direct supervision
  • Use a problem-solving approach
  • Demonstrates high standard of Infection prevention and control
  • Clinical skills – Administration of medications including injections, catheterisation, wound dressings; compression bandaging as per national wound care strategy
  • Confidence and ability to establish rapport with team, clients and other agencies
  • Computer Literacy and keyboard skills
  • Practice with the NMC Code of Professional Practice
  • Access to a motor vehicle for use at work and UK driving licence.
