We currently have a full-time opportunity available, working 37.5 hours per week.

In this role, you will contribute to the assessment, planning, delivery and evaluation of care for patients with stroke and a wide range of neurological conditions. You will work closely with therapists, healthcare assistants, medical colleagues and the wider multidisciplinary team to deliver holistic care that supports recovery and rehabilitation.You will be responsible for maintaining high clinical standards while ensuring patients and their families feel informed, supported and involved throughout their care journey. This is an excellent opportunity to develop specialist rehabilitation nursing skills within a supportive and experienced team.Key responsibilities include:

• Delivering safe, effective and compassionate patient-centred care
• Contributing to the assessment, planning, implementation and evaluation of individual care plans
• Supporting patients throughout their rehabilitation and recovery journey
• Maintaining accurate and contemporaneous patient records
• Communicating effectively with patients, carers and members of the multidisciplinary team
• Promoting patient independence and wellbeing wherever possible
• Maintaining a safe, clean and positive clinical environment
• Working in accordance with NMC standards and professional guidelines

The service operates 24 hours a day, 7 days a week, therefore flexibility to work a range of shifts including nights, weekends and bank holidays is essential.

• Registered Nurse with a valid NMC registration
• Passion for rehabilitation nursing and supporting patients to maximise independence
• Excellent communication and interpersonal skills
• Strong organisational and time management abilities
• Ability to work effectively both independently and as part of a multidisciplinary team
• Compassionate and patient-focused approach to care
• Confidence using IT systems, including Microsoft Office applications

• Experience within a rehabilitation, neurology, stroke or community hospital setting
• Knowledge of caring for patients recovering from stroke or neurological illness
• Experience working within a multidisciplinary environment
• Commitment to continuous professional development
