Main duties of the job
The Registered Nurse will work as part of the Integrated Care Team, delivering high-quality, holistic and family-centred nursing care to babies, children and young people with life-limiting and life-threatening conditions at the hospice or in their own home.
The post is contracted 22.5 hours per week, worked flexibly in accordance with a rota agreed with the Nursing Service Lead. Shifts run 9:00am to 5:00pm across all care settings. In term time, the post holder will primarily be rostered at weekends; during school holidays, shifts may fall on any day of the week, in order to meet service requirements.
Working autonomously and as part of the multidisciplinary team, the Registered Nurse will assess, plan, implement and evaluate evidence-based nursing care tailored to individual needs.

As part of the clinical care team, led by the Director of Clinical Services, you will be part of a multidisciplinary team providing symptom management, palliative and end-of-life care, as well as planned and emergency short breaks. As a Nurse you will be developed to lead shifts and support the wider team to ensure high standards of care are delivered and develop yourself. We provide complex care support while children and their families benefit from our range of therapies at home or within our wonderful woodland grounds.

Job responsibilities

Accountable to: Nursing Service Lead

Assess, plan, implement and evaluate individualised nursing care.
Deliver nursing care within the Hospice at Home and Day Care services.
Promote child and family-centred care.
Undertake nursing assessments and contribute to care planning.
Administer and manage medicines safely in accordance with policy.
Provide complex nursing care, including tracheostomy care, emergency tracheostomy management, deep suctioning, oxygen therapy, invasive and non-invasive ventilation (CPAP/BiPAP), gastrostomy care, nasogastric tube care and stoma care where competent. Training towards skills provided.
Recognise deterioration and escalate concerns promptly.
Maintain accurate electronic patient records.
Provide education and support to parents and carers.
Participate in safeguarding processes.
Maintain a safe clinical environment and practise in accordance with the NMC Code.
Work collaboratively with the multidisciplinary team.
Participate in handovers and MDT meetings.
Support Healthcare Support Workers, students and new staff.
Contribute to a positive team culture.
Participate in audit, quality improvement and incident reporting.
Ensure compliance with CQC standards and organisational policies.
Maintain NMC registration and continuing professional development.
Attend mandatory training, appraisal and supervision.
Support the education of staff, students and parents/carers.
Maintain professional boundaries and confidentiality.
Keep accurate records.
Demonstrate professionalism, integrity and compassion.

- Excellent communication and interpersonal skills
