A great opportunity has risen for a Registered Nurse within the Frailty SDEC team at University Hospitals Plymouth.
We are looking for an individual who is enthusiastic about working in the acute hospital setting with a compassionate, patient-centred approach. We will support the successful candidate in their personal development and continued professional development.
We provide a same day emergency care service to frail patients over the age of 65 years within a friendly team committed to providing high quality patient care.
Previous demonstrable experience within health care of the elderly care is an advantage but not essential as full training will be given. Preceptorship applications will be considered.
The successful candidate will work as part of the nursing team to provide high standards of care to ensure a positive experience to all patients.
***Preference will be given to internal Trust staff, as well as ‘Priority’ and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***
Main duties of the job
- Under the direction of the nurse in charge you will co-ordinate, and deliver effective quality nursing care by assessing the needs, planning, implementing and evaluating the care for a group of patients
- Work as part of the multidisciplinary team
- Following a period of preceptorship take responsibility for the supervision of Health Care assistants and other learners.
Detailed job description and main responsibilities
- Provide and deliver a high standard of nursing, care (i.e. washing, mouth care, and toileting), recognising and meeting the specific needs of patients.
- Provide and prioritise own tasks.
- To carry out clinical observations on patients, recognising, recording and reporting all changes in patients condition to senior staff.
- To support maintenance of fluid balance and dietary charts, recognising, recording and reporting all adverse findings to senior staff.
- Undertake the escort of patients to other departments, under the direction of the registered nurse (when deemed competent).
- To support nutritional needs of patients by assisting with feeding and drinking, reporting on concerns about dietary intake to senior staff.
- An awareness of skin integrity over pressure points, recognising and reporting all adverse findings to senior staff.
- To recognise the importance of health education and teaching of patients and carers, and refer patients to senior staff.
- Document and sign for all observations and care given
- To maintain his/her own professional development, attending courses/further training to enhance and optimise his/her role within the work area.
