Job overview
The postholder will support patients through their hospital care journey, providing the patient with a single named point of contact who will coordinate their care. Work as part of a team within a dedicated speciality/patient group to ensure that all administrative functions for patients from the time of GP referral to the time of discharge are coordinated in line with specific care pathway instructions to facilitate the smooth running of the patients’ journey. Where a patient has multiple specialist needs, assist the patient in coordinating their pathways to reduce multiple appointments across the Trust.
Main duties of the job
We are seeking a proactive and resilient Colorectal Pathway Navigator to support the effective delivery of the service, with a specific focus on supporting the Colorectal Lower GI Cancer performance at the NMH site. This role is central to ensuring patients move smoothly and safely through their diagnostic and treatment pathways, working closely with clinical teams across multiple sites.
This is a busy, fast paced position that requires excellent attention to detail, strong organisational skills, and a commitment to delivering a high‑quality, patient centred service. The post holder will play a key role in monitoring and improving pathway performance, ensuring cancer targets are met and escalations are managed appropriately at the NMH site.
The successful post holder will need to demonstrate the ability to work independently, manage competing priorities, and communicate confidently with both patients and clinical colleagues.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
