
Job summary
The post-holder will provide a comprehensive Administrative Service for the Complex Discharge Team based within our Complex Discharge Hubs.
All activities will underpin the safe and timely discharge of patients. As well as work to reduce patient length of stay
The role involves cross site working on a 7 day rota to include Bank Holidays.
Main duties of the job
Receive and process all referrals triggered by ward staff. To record and monitor the above information on the Discharge Hub Management System and any other systems/excel sheets so that the referral trail is auditable.
Provide intelligence on Complex Discharge Hub performance. Work will involve collecting/inputting to produce spreadsheet reports for the Complex Discharge Hubs, e.g. length of stay of patients on the Complex Discharge Hub caseload, discharge destinations, number of medically fit patients and other ad hoc reports.
Develop knowledge of the Discharge Hub Management System and use within the everyday role to support discharge planning for patients who have care needs following their acute stay.
Required to provide reports or take minutes as required.
Provide an administrative and secretarial service to the Complex Discharge Hub Team including maintaining a diary for the Complex Discharge Team.
Responsible for any supporting requests for patient notes made by the team using Case Note Tracking as required.
Required to order stationery/ equipment required by the team using the appropriate system, to raise requisitions and receipt invoices where applicable.

To receive all incoming calls, prioritising them accordingly and logging patient referrals when appropriate. Participate in the receiving and distribution of all internal and external mail, and logging and/or redirecting as appropriate.
To liaise with outside agencies and voluntary organisations on behalf of the Complex Discharge Team. To liaise with internal staff across ward area that includes all disciplines.

Essential
- Experience of working in an administrative environment, ideally in a Healthcare setting.
- Experience of working with the public/ customer service.
- Experience of Microsoft packages including Word, Excel, Outlook.
Desirable
- Experience of minute taking, preparing and distributing agendas for meetings.
- Experience of working in a busy environment.
Qualifications
Essential
- NVQ Level 3 in Business Administration or equivalent qualification and/or experience in an administrative environment.
- RSA level II or equivalent experience.
