
Job summary
Mayflower Medical Centre, on behalf of Stour Estuary PCN, are currently seeking an experienced advanced nurse practitioner to work within thePro Active Care Team across our PCN which also includes Ardleigh Surgery, Fronks Road Surgery, Harewood Surgery and Riverside Health Centre.
This role is offered on a fixed term contract until 31st March 2027, subject to continued funding and will be on apart time basis of 19 hours per week, mainly community based. This role will lead our Pro Active Care Team of social prescribers and GPA's and will involve pro active care and completing ReSPECT forms to enable patients to be supported in their own homes and avoid hospital admissions.
Main duties of the job
- Diagnosing and treating patients presenting to primary care with acute medical conditions, minor illness and mental health problems, including a mix of home visits and telephone consultations.
- Ensuring clinical practice is safe and effective and working within boundaries of competence.
- Initiating and arranging appropriate investigations and referrals to other members of the primary and secondary health care teams as clinically indicated.
- Participating in the administrative and professional responsibilities of the practice team including clinical audit.
- Ensuring the clinical record is completed and maintained with accurate and contemporaneous details recorded.
- Participating in the education and training of students of all disciplines and the induction of all members of the practice staff where appropriate.
- Actively participate in clinical and whole-team meetings (including morning briefings), as this promotes information sharing, supports effective communication and helps maintain positive and professional working relationships within the workplace.

We are a friendly, well-organised training practice looking after 17,600 patients in an impressive purpose-built GP surgery, located within a community hospital in the seaside town of Harwich in Essex.

- Strong interpersonal and communication skills and be able to liaise effectively with the clinical and administrative staff within the practice and partnership
- An ability to demonstrate innovation and awareness of future opportunities and challenges would be desirable to help meet the current and future challenges of primary care, and to help us promote a rewarding environment for staff as well as ensuring excellent patient care
- An independent prescribing qualification is preferred/but not mandatory
