We are looking for an experienced Advanced Nurse Practitioner (ANP) / Acute Frail Elderly Outreach Matron to join our friendly and professional team in serving our patients.

The successful candidate will be an experienced, motivated and enthusiastic ANP/Matron. The candidate must have the Prescribing qualification for this role.

The post is for full-time hours (or up to full-time hours). Working 3 days per week as an ANP and 2 days per week at a Matron.

Main duties of the job

Responsible for the clinical assessment, investigation, diagnosis and treatment and/or referral of patients within urgent care services, identifying complex and specialist needs and will develop, implement and evaluate programmes/plans of care.

Working as an ANP/Matron with a high degree of autonomy, the post holder will manage undifferentiated and undiagnosed minor injuries and minor illness, using advanced clinical skills within own sphere of competence

Providing direct consultations which dependent upon competency and area of practice, may be via telephone or face to face.

PURPOSE OF ROLE

Working as an Emergency Care Practitioner with a high degree of autonomy, the post holder will manage undifferentiated and undiagnosed minor injuries and minor illness, using advanced clinical skills within own sphere of competence.

The post holder will work collaboratively with the team and external providers, to ensure the right care, by the right person, at the right time and in the right place. Practitioners will aim to enable individuals to maintain or regain independence or ensure a plan is in place for them to achieve their optimum personal potential and to prevent unnecessary hospital admission.

The post holder will demonstrate a commitment to providing a quality, evidence based service, excellence in clinical practice, this together with continuous professional development are integral to the role and development of the service.

The service operates during practice opening hours, which may change depending on local guidelines.

MINIMUM QUALIFICATIONS AND EXPERIENCE REQUIRED FOR THIS POST

• Recognised professional degree or equivalent
• Postgraduate study at Masters level
• Registered with appropriate professional regulatory body
• Relevant post registration qualification in autonomous practice or equivalent.
• Evidence of relevant post registration experience
• Independent Prescribing or evidence of working to PGDs with advanced knowledge (AHPs)
• Evidence of continuing professional development
• Excellent written and verbal communication skills
• Computer literacy
• Knowledge of primary and urgent care issues

CORE COMPETENCIES REQUIRED FOR THIS POST

Competency Standard 1 - The roles of the Advanced Nurse Practitioner

The ANP is expected to:

• The post holder will take a lead role in the advanced clinical assessment, investigation, diagnosis, treatment and /or referral/discharge of a diverse and undifferentiated and undiagnosed problems, in accordance with professional standards and codes of conduct, national best practice and locally and nationally agreed standards.
• Will act as a credible expert, leading a team in the care of patients, who may have highly complex and or/ chronic presentation, developing, evidencing high-level assessment, assimilation of information implementing and evaluating personalised clinical management plans.
• Undertaking appropriate investigations to formulate a diagnosis implement and evaluate treatment/ interventions and care for patients presenting with an undifferentiated diagnosis, complex needs or long term conditions.
• Prescribing within the boundaries of extended non-medical prescribing
• The post holder will provide critical thinking and analytical skills in practice, and attainment of knowledge, and use their experience to support practitioners in the development of their own skills and knowledge.
• To act as a role model, resource and mentor, promoting excellence in practice, maintaining and monitoring standards of care, highlighting and working with others as appropriate to manage performance where there is cause for concern. Both providing and participate in clinical supervision and undertaking peer review of practice.
• Support the collation of the training needs analysis, promoting learning and creating a positive learning environment.
• Participate in quality improvement, supports the development of service improvements, and also utilises knowledge of clinical governance to provide safe and effective services to individuals.
• Provides support, advice, guidance and training to patients, families and carers, to promote the understanding and consent to care.
• Access's research and interprets research, identifies gaps in practice, utilises appropriate evidence bases to support practice development and applies research evidence into working practice.

Competency Standard 2 - Your personal development

The Emergency Care Practitioner is expected to:

• Produce, develop and maintain a Personal Development Plan, know where you can get personal support and be aware of how others can help review and prioritise your personal development needs
• Understand why feedback from others is important in helping to develop and improve the way you work
• Use learning opportunities and reflective practice to contribute to personal development whilst also understanding the importance of reflective practice to continuously improve the quality of service you provide
• Participate in clinical supervision as a means to improving the quality of care to patients.
• Acknowledge own limitations, consciously reviews practice (mistakes / omissions/successes) to improve performance, reports any concerns regarding patient care, Identifies risks which could affect patient care
• Participates in staff development review and continuous professional development
• Challenge poor practice and be open to challenges by others.
• Plan, provide and evaluate the learning environment for pre-registered, post registered nurses and others as applicable; participates in the professional development of healthcare support workers.
• Act as a supervisor, mentor or preceptor as required.
• Contributes towards eliciting own training needs.
• Demonstrate professional conduct as defined by your professional code of conduct preserving patient rights and promoting mutual respect amongst colleagues.
• Attend all mandatory training as identified by line manager
• Acknowledge own limitations, consciously review practice (mistakes / omissions/successes) to improve performance, report any concerns regarding patient care, Identify risks which could affect patient care
• Maintains and develops own knowledge and skill base, responsible for own professional development, ensuring possession of the right skills, to deliver the agreed level of care appropriate to meet the patient's needs.
