The role involves building and maintaining effective, professional relationships between hospital staff and community teams for children, young people, adults who need care, as well as their families and carers using services at Blackpool Teaching Hospitals NHS Foundation Trust. It also includes helping staff work together smoothly with both community and hospital services, ensuring timely coordination. Additionally, the position oversees the liaison model and makes use of resources available within the Trust’s corporate safeguarding team.

Conducting case reviews, and sharing identified themes and lessons learned every quarter related to the Emergency Department.

Supporting patient safety is crucial, with duties including timely incident reporting and ensuring that any resulting learning is widely shared. Providing highly specialist advice and support to the ED MDT and education/ training to other employees will be a key part of the role.

The position also includes line management of the Emergency Department (ED) Navigators and will work as part of the Trusts corporate safeguarding team.

Main duties of the job

  • Work autonomously organising; planning and prioritising own workload priorities for own area of work.
  • To demonstrate an understanding of Clinical Governance and Risk Management
  • Facilitating and supporting staff to efficiently coordinate with community and hospital services in a timely manner.
  • To undertake regular documented reviews of practice through audit, case discussion, peer review, and supervision and as a component of refresher training.
  • To work closely with ED ward managers, team leaders and Matrons and contribute to staff training and development including induction and mandatory programme safeguarding training.
  • Encourage case discussion, reflective practice and the monitoring of trends at a local level, incorporating safeguarding processes and pathways.
  • Provide management and safeguarding supervision to ED Navigators.
  • Communicate complex, sensitive, and sometimes contentious information with empathy, persuasion, and reassurance.
  • Act as a key link between emergency care teams, all age statutory safeguarding services, and community partners.
  • Support staff in making referrals to social services for children, young people, and adults at risk undertaking a “Think Family” approach.
  • Work closely with Named Professionals and Lead Nurses with the Trust corporate safeguarding team and line manager to ensure outcomes from audits are used to improve patient safety
