You will work closely with multidisciplinary colleagues across acute, community, primary care and social care services, helping to coordinate care, reduce avoidable admissions and support the delivery of Discharge to Assess and Home First pathways. This is an exciting opportunity to use your clinical assessment, decision-making and communication skills within a fast-paced, rewarding environment focused on improving patient outcomes and experience. We are looking for a motivated and experienced registered healthcare professional who thrives on collaborative working, values innovation and is committed to delivering high-quality, person-centred care. In return, we offer opportunities for professional development, multidisciplinary learning and the chance to make a meaningful difference to patients, carers and the wider health and care system.

At present, the Trust is unable to offer Skilled Worker visa sponsorship for this post. Applicants must have the right to work in the UK at the time of application.

Main duties of the job

Support the delivery of same-day discharge pathways from Urgent and Emergency Care areas, contributing to improved patient outcomes for individuals with a wide range of health and social care needs, including complex presentations.

Responsible for identification and assessment of patients who may be appropriately supported by community services, contributing to the avoidance of unnecessary admission to acute beds.

Work part of the Transfer of Care Hub (TOCH), collaborating with multidisciplinary colleagues across acute, community, and primary care settings, as well as Adult Social Care and partner agencies, to support effective partnership working and the coordinated discharge of patients, including those with more complex needs.

Act as a specialist resource within the acute setting, utilising knowledge of community services to support safe, timely, and effective discharge planning, including facilitating access to appropriate services where required.

Ensure patients are appropriately signposted and supported to access follow-on community services, contributing to the reduction of avoidable readmissions and ensuring safe continuity of care.

Ensure accurate, timely, and clinically appropriate communication and handover of care to community services, contributing professional judgement to inform discharge planning and next steps.
