Job overview

An exciting opportunity has arisen to join the Integrated Discharge Team (IDT) and Transfer of Care Hubs as an Administration and Clerical Officer. The successful candidate will provide an efficient and responsive administrative and clerical service, including the processing of referrals and supporting the coordination of complex discharge planning.

Working closely with Health and Social Care colleagues across Doncaster and Bassetlaw Teaching Hospitals, you will play a key role in supporting patient flow and ensuring timely, effective discharge arrangements in a fast-paced environment.

We are seeking a dynamic, organised, and adaptable individual who is flexible to work across both Doncaster and Bassetlaw Hospital sites on a rotational basis. In return, you will join a friendly and supportive team committed to achieving the best possible outcomes for patients and their families while continuously improving efficiency throughout the patient journey from admission to discharge.

Main duties of the job

A high level of multitasking involved in this role.

Attending Multidisciplinary meetings, documenting outcomes on electronic systems.

Sending referrals via email.

Updating Live Reporting systems.

Answering telephone enquiries.

Supporting all members of the Transfer of Care Hub/Integrated Discharge Team with general Clerical/Administration tasks.

Working across Doncaster and Bassetlaw Hospital on rotation

Communicating planned patient discharges with the hospital site team, ward staff and relatives.
