An exciting opportunity has arisen within NHS Fife’s Acquired Brain Injury Outreach Nursing Team (ABION). We are seeking a highly motivated, compassionate and skilled Registered Nurse to join our team. It is a very rewarding role, supporting adults living with the consequences of an acquired brain injury and enabling them to achieve the best possible outcomes.

As an ABI Outreach Nurse, you will:

• Provide specialist nursing assessment, care planning and intervention for individuals with acquired brain injury
• Work closely with patients, families and carers to support rehabilitation, adjustment
• Deliver care across a range of community settings, including patients’ homes and outpatient clinics
• Contribute to multidisciplinary team working alongside AHPs, medical colleagues, social care and third sector partners
• Provide specialist information and advice on issues related to acquired brain injury to other professionals within health and social care, including voluntary and statutory services and GP’s, acting as an expert clinical resource.
• Support seamless transitions between inpatient services, community care and other agencies
• Promote health, wellbeing, self‑management and risk reduction
• Act as an advocate for people with ABI, ensuring care is delivered in a holistic way
• Evaluate and audit practice to improve and develop the ABION service.

The post holder will be a Registered Nurse with significant post registration experience of working with people with a neurological condition in a rehabilitation environment, ideally with a proven interest in working with people with an Acquired Brain Injury. You will be educated to degree level and be confident in autonomous practice, skilled in assessment, communication and relationship building with a person centred approach to care.

The post holder is currently based at Fife Rehabilitation Service at Cameron Hospital and requires flexibility to travel between patient's homes and community settings
