JOB DESCRIPTION

JOB TITLE: Support Worker

GRADE: Band 4

BASE: Tyrone & Fermanagh Hospital, Omagh

REPORTS TO: Sector Team Lead

RESPONSIBLE TO: Team Co-ordinator

JOB PURPOSE AND SCOPE

RISE NI is a regional early intervention service which supports children in pre-school education and mainstream primary school (Year 1 to Year 4 inclusive) settings by working closely with parents and education staff to help children develop the foundations skills for learning, i.e. speech, language, communication, sensory-motor, visual perception, social skills, emotional wellbeing and behaviour. The transdisciplinary team includes Speech and Language Therapists, Occupational Therapists, Physiotherapists, SEB (Social, Emotional and Behaviour) Practitioners, and Support Workers.

The post holder will support the provision of a flexible, child-centred service to children and families by delivering individual intervention programmes, modelling sessions, group interventions and clinical tasks as delegated by clinical staff. RISE NI interventions generally take place in the child’s mainstream education setting.

MAIN DUTIES & RESPONSIBILITIES

Clinical

1. Responsible for providing a range of high quality group and individual intervention programmes to identified children and young people as delegated by RISE clinical staff.

2. Implement agreed intervention plans in relation to parenting support and home programmes in the home environment as delegated by the RISE clinical staff.

3. Advise on and reinforce aspects of the intervention programmes to children, carers and teachers as directed by RISE clinicians.

4. Responsible to modify interventions within agreed therapy programmes as required and within scope of practice and delegation arrangements.

5. Support delivery of the service through preparation of required resources and equipment for intervention programmes as directed.

6. Make safe a range of environments in the delivery of programmes.

7. Work collaboratively within the transdisciplinary team and with education staff to ensure the effective delivery of this service to children.

8. Manage and prioritise workload and diary including direct clinical activity, meetings, and documentation.

Communication

1. Maintain confidentiality with regard to client information.

2. Demonstrate appropriate and effective communication with children, some of whom may exhibit challenging behaviour and / or speech and language and communication difficulties.

3. Report effectively to the keyworker / team on the child’s performance and progress.

4. To demonstrate effective and professional communications skills with the carers, education staff, members of other health and social care teams and team colleagues.

Documentation

1. To keep accurate and up-to-date clinical records adhering to departmental guidelines.

2. Assist the team in the recording of data and participate in clinical audit as required.

3. To share information in accordance with Professional and Trust standards.

4. To ensure security of records and information.

Service Development and Delivery

1. To be responsible for the maintenance and the orderly storage of equipment.

2. To ensure that work and storage areas are well maintained and comply with health and safety guidelines ensuring the safe use of equipment and storage of materials.

3. To participate in the planning and delivery of the team’s development and service delivery plan.

4. Attend and participate in team meetings as required.

5. To demonstrate role and share knowledge and skills with students, work experience students and new staff.

Professional Development

1. To continuously develop knowledge and skills relevant to this post through in-service training, courses, etc. as agreed with the line manager.

2. Maintain a record of own training and development activities.

3. To review and reflect on own practice and performance through effective use of professional and trust standards.

1. To comply with a code of ethics and Professional Conduct and national and local procedures.

Other Duties

1. Consistently contribute towards maintaining a positive environment and maintaining good team morale. Communicate efficiently and effectively with all team members and neighbouring teams.

4. Co-operate with Trust Policies and comply with RISE NI local guidelines and procedures adhering to all deadlines.

5. Co-operate with annual leave, time in lieu (TIL) and other agreed arrangements within the team.

6. Assist with the induction of new staff as and when required.

The post holder will be working in an ever evolving and developing service and therefore is expected to adapt to change and undertake any other duties with the grade of the post as required. The execution of other duties (as appropriate to the grade) may be assigned from time to time in order to meet the needs of the service and ensure the smooth running of the team.

GENERAL DUTIES
