An exciting opportunity has arisen to join the Shropshire, Telford & Wrekin Urgent Community Response & Virtual Wards service as a Community Physiotherapist. The post will form part of a seven day service based within one of the Trust’s locality teams. The service provides patients who are experiencing an acute change in their health a safe and effective alternative to hospital admission, supported by a mixture of face-to-face visits and digital technology.

We are looking for a Physiotherapist who shares our passion for providing excellent quality of care and can help deliver a recommended and agreed treatment/care plan to service users in their own home. The post holder will have full access to the Multidisciplinary Team including consultant, GP, Pharmacist, Nurses, Therapists, and Health Care Support Workers.

Main duties of the job

You will need excellent assessment and intervention skills in order to plan, develop and evaluate goal focused rehabilitation and reablement programs, as well as complex discharge planning experience. An understanding of Health and Social care provision and ability to work closely with other organisations and stakeholders is essential.

You will need to demonstrate high standards of evidence based clinical practice. You must have excellent interpersonal and communication skills. You should be able to prioritise your workload, manage your time effectively, and work flexibly within the team.

We are looking for enthusiastic, motivated, and conscientious & experienced Physios to work with the established therapy teams. We are a dynamic, developing organisation with well recognised rehabilitation services providing education, advice, support and rehabilitation for patients.

  • Must be able to meet travel requirements of the post.

Experience

  • Significant post graduate experience in a variety of clinical settings with experience in a community setting.
  • Experience of working in Multi-disciplinary/Multi-agency teams.

Skills and Knowledge

Essential

  • Understanding of Clinical Governance and risk Management and their Implications for the service.
  • Ongoing Awareness of recent developments in practise including NICE guidelines.
  • Excellent time management skills.
  • Excellent interpersonal and communication skills.

Qualifications & Training

Essential

  • Diploma/Degree in Physiotherapy.
  • Current registration with HCPC.
  • Evidence of specialist Post Graduate training and Education with older adults.
  • Evidence of core rotations (MSK, orthopaedics, neurology, respiratory, care of the older adult).
