Working with an experienced multi-disciplinary team, you’ll provide urgent assessment and treatment so that patients receive safe, timely care when they need it most.
City Health Care Partnership CIC is looking for a skilled and compassionate practitioner to support the delivery of high-quality urgent care services. You will assess and treat patients of all ages presenting with minor illness, minor injuries, acute conditions and exacerbations of long-term health needs. This role offers the opportunity to work across a range of healthcare settings, supported by a well-established clinical team and ongoing professional development.
Main duties of the job
- Assess, examine, diagnose and treat patients, or refer them to the most appropriate service
- Deliver safe, effective and evidence-based care for patients with urgent healthcare needs
- Work closely with colleagues and partner organisations to ensure seamless patient journeys
- Support and guide junior staff while acting as a positive role model
- Contribute to service improvement through audit, research and the use of clinical guidelines
- A registered clinician with experience managing minor illness and/or minor injury presentations
- Strong clinical assessment and decision-making skills
- The ability to work within professional boundaries and follow policies and procedures
- Effective communication, leadership and teamwork skills
- A commitment to continuous learning and delivering excellent patient care

You must have appropriate UK professional registration.
This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
Person specification
Knowledge
Essential
- Significant knowledge of relevant policy and legislation, guidance and local initiatives and issues in relation to unplanned care
- Working knowledge of multidisciplinary teams
- Evidence of Continuing Professional Development
- Demonstrate the use of evidence based practice
Desirable
- Comprehensive working knowledge of medication and interventions used in unplanned care
- Knowledge of the legal framework surrounding medications prescribed or supplied, including PGDs, PSDs, EOL care pathways etc.
Experience
Essential
- Experience in an acute or primary care setting
- Experience of workload management and the ability to meet deadlines
- Experience of utilising a variety of software packages e.g. SystmOne, and E-Mail
Desirable
- Previous experience of using PGDs
- Experience of multi-disciplinary team working and working with patients with minor injuries and minor illness presentations
- Experience of the initial management, stabilisation and onward referral of patients who may self-present with more serious/life threatening conditions
Qualifications
Essential
- Degree in Nursing or Paramedic Science (Level 6) or significant experience.
- Academic modules / specialist training completed in relevant area/subject
- Professional Registration with the NMC (Nursing and Midwifery Council) or HCPCP (Health Care Professional Council)
