The Home Treatment Service (HTS) is part of the Urgent Care Service, providing hospital-level assessment and treatment in patients' usual place of residence. Supporting people with frailty, long-term conditions and acute deterioration, the service helps avoid unnecessary hospital admissions and enables timely discharge from hospital.
As a Band 7 Urgent Care Practitioner, you will deliver advanced clinical assessment and treatment through planned and urgent home visits, working across a 7-day service. Using advanced history-taking, physical examination, clinical reasoning and risk assessment skills, you will autonomously assess, plan, implement and evaluate care for patients with complex and changing needs. Working closely with acute trusts, primary care, ambulance services and community partners, you will coordinate person-centred care, make timely clinical decisions and contribute to multidisciplinary planning to achieve the best outcomes for patients and carers.
Main duties of the job
We are seeking an experienced and motivated clinician who can combine expert clinical practice with leadership, education and service development.
You will provide senior clinical support to colleagues, supervise and develop staff and students, and act as a role model for evidence-based practice and professional standards.
Alongside delivering high-quality care, you will contribute to governance, audit, quality improvement and pathway development, helping to shape and improve urgent community services. You will be confident managing a dynamic caseload, prioritising clinical need, responding to changing demand and supporting operational service delivery.
A commitment to safeguarding, excellent communication, multidisciplinary working and continuous professional development is essential. In return, you will join a supportive, innovative team delivering responsive care that enables patients to remain safely at home whenever possible.
