This post will triage referrals into the Gynaecology service utilising approved triage criteria and tools, supported by the Lead Triage Nurse  and the Consultant team, the post holder will be expected to accept, reject, upgrade and downgrade referrals into the service.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Main duties of the job

To act as the first point of contact when a referral comes into the service from primary care or another specialty. This will include the management of all referrals determining the level of urgency and appropriateness of the referral with the support of the Lead Triage Nurse, decisions will be complex as they will relate to people experiencing a variety of gynaecological symptoms with varying degrees of severity and the role will be demanding due to it being fast paced.

Detailed job description and main responsibilities

To work within the guidance of the Lead Triage Nurse and Consultant colleagues, establishing good relationships with  primary care partners.  • To conduct assessments for clients with complex needs and formulating robust and achievable collaborative care plans or  introduce appropriate alternative pathways.  • To work within the policies, procedures and good practice guidelines set out by the ABUHB and professional bodies.   • To support the multidisciplinary team in developing services that are sensitive to the individual needs of patients.  • To establish and maintain effective care through the holistic assessment of a patients needs, working in a safe and  accountable fashion, including participating in audit.   • To promote a value base service which meets the needs of empowered service users by ensuring that their wishes to live  and experience an ordinary life are respected and supported.  • To have the interpersonal skills necessary to develop relationships with colleagues, other agencies including primary care  and patients.   • To complete comprehensive risk assessment and patient focussed plans in accordance with Aneurin Bevan University Health Board (ABUHB) policy and guidance.  • To prioritise need and workload with the Lead Triage Nurse ensuring effective use of time and professional accountability.  • To provide clinically effective, clinically focused and an evidence based service for patients.   • To advise on the promotion of prevention of poor health.

Experience

Essential

  • Relevant experience at Band 5 or equivalent
  • Evidence of working as part of a multi-disciplinary team
  • Engagement with service audits
  • Clinical supervision
  • Previous experience of complex risk management
  • Previous experience of case management
  • Excellent organisational skills
  • Able to work well under pressure
  • Ability to prioritise workload

Desirable

  • Evidence of supervising and developing Junior Staff
  • Evidence of team leading junior staff
  • Evidence of leading change or new initiatives or other service development

Qualificiations and Knowledge

Essential

  • Registered Nurse - Degree level
  • Additional training and/or experience and/or enhanced practice to post graduate diploma level equivalent
  • Extensive experience in Gynaecology
  • Previous training in Risk management
  • Evidence of continuous Professional Development

Desirable

  • High level training in Risk Management
