Job summary

This is a vital role focussing on the transition between paediatric and adult renal services, caring for 1625-year-olds. The post is based at SJUH but has close professional links with the paediatric team at the LGI and the wider renal team, including satellite dialysis units across West Yorkshire. Transition and young adult care can be challenging but is balanced with the rewarding nature of the role. The role is all about supporting young adults to move seamlessly from childrens services to adult services, co-ordinating their care, managing your time efficiently and prioritising the workload. There is a need for a current registration with the NMC and at least 2 years experience in relevant fields of nursing.

Main duties of the job

You will support patients in a way that is impactful to their care. You will be the main point of contact for young adults for queries or concerns, or someone to talk to during the challenges of being a young person balanced with managing a chronic condition. Flexibility with your diary is essential when managing your time to fit around the differing needs of your caseload. You will have the support of the wider team to create MDT meetings regarding your caseload which requires good communication. Care for your young adults will be a mixture of text/WA, telephone and face to face. You will need to ensure your young adults feel advocated for and supported in adhering with their care, as well as being encouraged to pursue their goals as a young person. This may mean communicating with families or carers of that young person to ensure you are aware of how best to support them. The renal CNS service is looking for a confident, enthusiastic individual who incorporates the Leeds Way Values and applies them to their work. The renal service is made up of hard-working individuals who are supportive and friendly, sharing the same goal to provide the best care for their service users. Transition and Young Adult care is the future of long term care and there is a great interest and drive to improve this year upon year.

As well as colleagues in the renal service on hand to support you, there is external support from psychology and Kidney Care UK.
