We are looking for a registered nurse to be part of a Multi disciplinary team, identifying and case managing patients with complex discharge needs. You will be part of a team working across Pilgrim County Hospital site, assessing and facilitating timely planning of discharges. You will be working with Patients, families and ward base staff to implement a strength based approach to discharge planning. This role involves working closely with allied professionals, leading MDT meetings and supporting our discharge coordinators in their role.

Main duties of the job

You will be responsible for the discharge planning for patients with complex discharge needs.

You will act as advocate for patients and carers, ensuring individual needs are met.

Ensure safe discharges of all patients .

You must have appropriate UK professional registration.

Previous experience

  • Completion of preceptorship
