Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

Working closely with health and Social Care colleagues to support the complex discharges out of the acute hospital to ensure the level of service is appropriate to meet their needs.

Provide a holistic approach to interventions, working with the whole person, their family or system, building relationships and networks, enabling individuals, their families and carers to achieve sustainable good outcomes.   Have a good understanding of KCC law and procedures or know how to access this information/advice if required.

Support and participate in, joint and integrated working with a range of teams including community safeguarding teams, partner organisations and other parties to offer a broad range of options to clients and carers to keep them safe and reduce risk. Join wash up discussion within health to encourage sharing of knowledge and information.

Main duties of the job

• This is a busy role so you will be working together within your team to balance different tasks that often change.
• Working as part of a team and on your own, you will draw from your knowledge, skills and experience, to make informed decisions and use your initiative. You may also be required to work in collaboration with multiagency partners.
• You will play a key part in safeguarding people by recognising and responding when an adult or child might be at risk from abuse but also when necessary. This will also include assessment of capacity and assisting with decision making.
• You will be required to maintain clear and accurate records of all advice provided to families, observations and action taken.
• You will be required to assess, plan, agree, implement and evaluate care for patients, recognising your own limits and asking for help and escalating concerns. You will also be responsive to changing in a patients condition.
• We have a number of policies and procedures in place to support you in keeping both yourself and our patients safe. You will also have the opportunity to comment on and be responsive to any proposals for change and participate in audits and surveys.
