An exciting opportunity has arisen for Band 4 Assistant Therapy Practitioners to join our Community Admission Avoidance Therapy Team (CAATT). This post will be based at the Royal Gwent Hospital, working Monday- Friday.

The post holder will contribute to the provision of CAATT by providing high quality, effective support to people who have presented to the emergency care department/acute medical units. This activity supports the ABUHB Clinical Futures work aimed at preventing unnecessary admissions to hospital, reducing length of hospital stay, providing care closer to home and establishing home-based support where appropriate by liaising with primary, community and third sector organisations

Main duties of the job

This role is recognised as a key part of the multi professional service delivery and we are looking for skilled, organised and enthusiastic individuals to join us. Patient management will include responsibility for the delivery of therapeutic interventions as delegated by the registered therapists along with close liaison with patients, family, members of the MDT and external agencies. Post holders will need to demonstrate good time management, prioritisation skills and flexibility in their approach as well as the ability to communicate effectively both verbally and in writing.

Successful candidates will work alongside registered Occupational Therapists, Physiotherapists and Dietitians and will manage a delegated caseload of patients. The role will include supporting the assessment and intervention plans of people with a range of functional difficulties within the bounds of the practitioner role, including those presenting with multiple co-morbidities, frailty and cognitive deficits

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

We provide all of our staff with the opportunity to develop the skills required to meet the needs of the people that we serve and provide innovative and effective ways of delivering therapy intervention across the professional boundaries of Dietetics, Occupational Therapy and Physiotherapy. We support all staff with professional supervision, regular development reviews and training opportunities.

Whilst this post will be based at the Royal Gwent Hospital, there is an expectation for staff working in the CAAT service to be flexible (with notice) to maintain CAATT service provision across the sites we provide service to.

Applicant requirements

This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.

  • Experience of working in a support worker role
  • Experience of working in the NHS

  • Previous experience of OT/Physio/HCSW roles
  • Car driver with access to vehicle

Qualifications and Knowledge

  • Numeracy and Literacy skills – English and Maths GCSEs C or above
  • Evidence of AGORED level 3 or equivalent

  • Evidence of understanding of role and admissions avoidance ethos
