Are you keen to work in a Multidisciplinary Community Therapy Service, helping patients work towards their personalised goals?
If so, we have a brilliant opportunity for an Assistant Practitioner to come and join us here in the District Therapy Team at Bolton Foundation Trust. We work across six neighbourhoods in Bolton to help provide a holistic community-centred provision.
We are seeking to appoint a dynamic, confident, innovative and flexible individual with excellent communication and organisational skills.
The service operates 7 days, 8am-6pm. At present, Band 4's do not take part in the weekend service.
** PLEASE NOTE** Applicants must have Level 5 Higher Apprenticeship or FdA in Health and Social Care Qualification. Qualified Physiotherapists need not apply.
Main duties of the job
As a Band 4 Assistant Practitioner you will be independently assessing and treating patients and managing your own caseload.
Working as part of a team whilst also being an autonomous worker, you will help patients achieve their personalised goals.
Carry out specific care and treatment interventions and adjust/develop as indicated with supervision as required from qualified staff in a variety of settings.
Supervising Junior staff as appropriate.
Detailed job description and main responsibilities
The successful post holder will work across a variety of disciplines, contributing to the assessment, planning and implementation of treatment and interventions to provide the care and rehabilitation of patients.
You will be a key player in providing assessment, personalised care planning and management of care to patients who may require intensive home support as an alternative to hospital care.
You will have the skills to progress and discharge patients from the service under in direct supervision of a registered therapist as required.

Knowledge
Essential
- Awareness of confidentiality, dignity and respect.
- Basic understanding of relevant medical conditions and appropriate therapeutic interventions
- Knowledge of how disability/impairment affects function.
Experience
Essential
- Previous experience working in community or hospital settings within the NHS
- Experience of providing therapeutic interventions to individuals and groups and adapting as required.
- Experience of working as part of a multidisciplinary team
- Previous experience working within the AHP services
Desirable
- Experience of working within a Community Therapy service
- Previous experience of working as a band 3 or higher, Physiotherapy or Occupational Therapy assistant
